Objective: To observe and contrast the influence of small doses ofpenehyclidine hydrochloride (PHC) on labor analgesia, stages of laborand mum-fetal.To provide the experimental basis for penehyclidineapplication to labor analgesia.Methods:100puerperas,ASA physical status1or2who selected andsuited for the labor analgesia were asked to participate in the study. If the puerpera were diagnosed cephalopelvic disproportion or latentphase prolong obviously, then they were excluded from the study.Patients were randomized assigned to two groups: penehyclidine group(experiment group) and control group.Experiment group were givenpenehyclidine intramuscular and control group were given salinebefore epidural anesthesia and after the puerpera were in the latentphase started regular uterine contraction. When the cervix dilatationwas3cm in the first stage of labor all the patients of each groupwere administered continuous epidural anesthesia, selected the L2~3orL3~4as puncture point, employed mixed liquor of ropivacaine andfentanyl as anaesthetic.Observe and record the change of time aboutthe first second and third stage of labor, fetal heart rate and theApgar score. Results: No significant difference was detected in the time of secondand third stage of labor, cesarean section rate, neonatal asphyxia rateand the amount of postpartum hemorrhage of the two groups. The first stage of labor was significantly shorten in penehyclidine groupcompared with control group. Conclusion: In the course of the Labor analgesia,Penehyclidine canshorten the time of first stage of labor effectively then the timeof stage of labor was shorten consequently and reduce the adverseeffect on mum-fetal because of prolong of the stage of labor in thisstudy. In the course of the Labor analgesia,Penehyclidine has no effecton the second and the third stage of labor and does not increase theamount of postpartum bleeding.
