Objective:Periodontitis is an infectious disease which involved with the periodontaltissue. The development of periodontitis is heavily influenced by the results ofthe interaction of periodontal pathogens and the immune response of the body.Helper T cells (Th) could support other immune cell differentiation and adjustthe immune response. The subjects were the patients with periodontitis. Testthe levels of Th1/Th2/Th17in peripheral blood (PB) with flow cytometry(FCM), and test the levels of Interleukin-17(IL-17) in gingival crevicular fluid(GCF)with the method of enzyme-linked immunosorbent assay (ELISA).Thenwe could understand the changes of the immune indexes, and provide theimmunological basis for the onset and development of periodontitis. Therelationship between IL-17and the clinical periodontal indices was analyzed,to discuss the role of IL-17in periodontitis pathogenesis.Methods:The periodontitis were patients selected from the Department ofstomatological of the Department of Oral Medicine of Hebei MedicalUniversity; patients of CP group were aged form18to50; patients of AgPgroup were aged form18to35.On the basis of their oral condition patients were divided into3groupswith11patients in the group of chronic periodontitis (CP),8patients in thegroup of aggressive periodontitis (AgP), and10patients in the normal group.The periodontal conditions of every tooth were detailed record as well as thenumber of missing teeth except the third molar. The clinical periodontalindices include the plaque index (PLI), the probing depth (PD), the attachmentloss (AL), the bleeding Index (BI) and so on. Meanwhile everyone took anoral pantomography. 1. Detect the levels of Th1/Th2/Th17in PBTo collect every patient2ml peripheral venous blood with heparinanticoagulation on an empty stomach early in the morning, and then detectedthe leukocyte count (include polymorphonuclear, lymphocyte, eosnophils andbasophilic granulocyte).Every sample established2portions, one was the test tube and the otherwas the negative control. Joined the corresponding antibody into the test tuberespectively. And joined the negative control antibody into the negativecontrol tube. Then joined100ul blood into every tubes. At last, detected thelevels of Th1/Th2/Th17by FCM.2. Detect the levels of IL-17in GCFEvery subject chose6teeth (16,11,26,36,31,46) for the index teeth,then took the mean value of their periodontal conditions. Using the filter paperstrip to collect the GCF, and detected the levels of IL-17by ELISA. Then wecould explore the relationship between IL-17and the clinical indices.Results1. The blood routine examinations of all the three groups were in thenormal range.2. The changes of immune indexes in the peripheral blood:Th1, Th2and Th17in the normal group were0.48±0.26%,1.49±0.70%,0.43±0.32%.In the CP group they were2.46±0.49%,0.62±0.35%,1.64±1.15%.Th1and Th17were significantly higher than normal group, meanwell, Th2was lower(P<0.01). In the AgP group they were2.37±1.07%,0.57±0.15%,2.12±1.09%.Th1and Th17were significantly higher than normalgroup, Th2was lower (P<0.01), but they had no significant difference fromCP group (P>0.05).3. The change of IL-17in GCFThe levels of IL-17were337.57±42.77pg/μl in the normal group, andthey were428.68±54.92pg/μl in the CP group,460.9±41.95pg/μl in the AgPgroup.IL-17in CP group were significantly higher than normal group (P<0.01).What is more, there was a positive correlation between theconcentration of IL-17and PD(r=0.924, P<0.01), as well as AL (r=0.930,P<0.01).IL-17in AgP group was significantly higher than normal group too(P<0.01).And there was a positive correlation between the concentration ofIL-17and PD (r=0.857, P<0.01).But there was no significant correlationbetween the concentration of IL-17and AL (P>0.05).To compare with the CPgroup, there was no significant difference (P>0.05).Conclusions1. Th2in both CP group and AgP group was significantly lower thannormal group, but Th1and Th17were higher. The immunologic functionreduced differently.2. IL-17in the GCF of patients with periodontitis was higher than normalgroup. With the increased of PD and AL the level of IL-17rises. It couldreflect the degree of periodontal tissue destruction.3. The Th1/Th2/Th17in PB and IL-17in GCF of both CP group and AgPgroup had no significant difference with each other. They had the same state ofimmune function.
