Objectives: To compare the clinical outcomes of different surgical positions includingsemi-lithotomy and general supine position combined with traction table applied inclosed reduction and internal fixation by proximal femoral nail anti-rotation (PFNA)for femoral intertrochanteric fracture patients.Methods: The clinical data of64cases of femoral intertrochanteric fracture patients ina northern hospital from January,27th,2009to March,15th,2013were retrospectivelyanalyzed. Based on the surgical positions,33cases were divided into general positiongroup and31cases were semi-lithotomy group. Operation time, proportion ofconversion to open reduction, morbidity of complications related to traction table andsurgical position and other indexes were recorded. Statistical analysis was used tocompare the difference of the above indexes between these two groups.Results: There were no differences between two groups in demographic informationlike age and sex, injury mechanism, fracture type and accompanied underlyingdiseases. Compared with supine position group, semi-lithotomy group presented ashorter operation time and lower proportion of open reduction conversion, withsignificant difference (P<0.05). Semi-lithotomy may reduce the morbidity ofcomplications related to traction table and position. However, it remainedindeterminate about the morbidity difference between two groups for the small samplevolume in the present study.Conclusions: During the operation of closed reduction and internal fixation via PFNAwith distraction by fracture table for treating femoral intertrochanteric fracture,semi-lithotomy can provide convenience for surgical procedure and intra-operativeX-ray examination so that it consequently reduces the operation time, open reduction proportion and the morbidity of complications related to traction table and surgicalposition. Such a surgical position is worth generalizing.
