Objective:through clinical randomized controlled study, observation "sofren injection" preventive and therapeutic effect of reconstruction of posterior myocardial reperfusion injury in patients with acute myocardial ischemia, and explore the possible mechanism of its effect.Methods:For "79cases of patients with acute chest pain patients" and admitted, in accordance with the inclusion and exclusion criteria, according to the condition, the informed consent of patients underwent SCAG, there will be more than one degree of coronary artery stenosis≥50%were randomly divided into blank control group, the creatine phosphate sodium treatment group, sofren injection therapy group, creatine phosphate sodium combined with sofren injection treatment group, no significant difference between the four groups of patients with basic situation. The blank control group:Patients with preoperative oral aspirin enteric-coated tablets300mg and chloride and Gray300mg, postoperative given aspirin and clopidogre0.1g/days,75mgV days, at the same time combined with low molecular weight heparin subcutaneous injection of5000U,1/12h,7days out of low molecular weight heparin. The sofren injection treatment group:blank group on the basis of the treatment, postoperative sofren injection of10ml into5%GS solution250ml intravenous drip,1times/day,5days a course of treatment. Group of creatine phosphate sodium injection drug treatment:the blank group on the basis of the treatment, postoperative creatine phosphate sodium injection0.5g in10%fructose250ml intravenous drip,1times/day,5days a course of treatment. The combined therapy group:blank group on the basis of the treatment, postoperative soffen injection of10ml5%GS solution is added into the250ml by intravenous drip, creatine phosphate sodium injection0.5g in10%fructose250ml intravenous drip,1times/day,5days a course of treatment. In the case of revascularization before surgery,3days take peripheral blood serum were detected, preparation, analysis and comparison of detection of myocardial enzymes, cardiac markers, NO, SOD, CRP, ET, TXA2, MDA index.Results:①reducing oxidation role:the treatment group compared with the control group, SOD was decreased after PCI treatment group, SOD patients decreased less than patients in the control group, the comparison between the two significantly different (P<0.05); among the treatment groups before and after treatment there were significant differences (P<0.05). The anti-inflammatory effects:the medicine group and the control group, increased CRP level after PCI, the elevated level of medication groups of patients with CRP than patients in the control group, the comparison between the two significantly different (P<0.05); compared with existing statistical differences in the treatment group before and after treatment in the CRP group (P<0.05).③reduce myocardial injury:the treatment group and the control group, CK-MB, cTn I were reduced after PCI, the elevated level of medication group with CK-MB, cTnI is less than the patients in the control group, the comparison between the two significantly different (P< 0.05); among the treatment groups before and after treatment there were significant differences (P<0.05).Conclusion:treatment group and control group patients after PCI were different degree of myocardial ischemia reperfusion injury. Compared with the control group, treatment group with lower degree of SOD, CRP, CK-MB and cTnI elevated significantly lower than control group patients. Display sofren injection and creatine phosphate sodium MIRI is reduced to a certain extent, may be one of the mechanisms of antioxidant, inhibit inflammatory reaction and decrease myocardial injury. Through observation of clinical cases confirmed, on the basis of conventional therapy plus sofren injection or sodium phosphocreatine, can relieve the syndrome patients with PCI after myocardial ischemia reperfusion injury of acute coronary artery, curative effect is better than that of Western medicine, can be used in clinical practice.
