The Clinical Research Of Effect Of Three-step Cycle Therapy Of Traditional Chinese Medicine Combined With Improved XELOX Scheme In The Treatment Of Ⅳ Period Colorectal Cancer

The study bases on the theory of Three-step cycle therapy of Traditional Chinese Medicine(TCM), designed by professor You according to the regularity of toxic and side-effects in the treatment of gastrointestinal cancer with chemotherapies.We take Three-step cycle TCM therapy combining the treatment experience of professor You to cure the patients of IV period colorectal cancer.Object:To observe the effect of Three-step cycle TCM therapy combined with improved XELOX scheme on chemotherapy poisonous side reaction and body tolerability in treating advanced colorectal cancer. And the indexes of clinical efficiency, such as clinical symptom amelioration, Karnofsky score, quality of life, the level of tumor markers, cellular immune function and the cancer objective efficiency, also have been observed in order to assess the clinical efficacy comprehensively. Thereby to provide the clinical evidence to spread using Three-step cycle TCM therapy and provide theoretical supportment and new method in the advantage of TCM in comprehensive treatment of tumors.Methods:There are53patients of advanced colorectal carcinoma have been randomly assigned into two groups:test group in which27cases were treated with Three-step cycle TCM therapy and improved XELOX chemotherapy, and control group in which26cases were treated with improved XELOX chemotherapy alone, with a course of112days. The chemotherapy poisonous side reaction, body tolerability, clinical symptom amelioration, Karnofsky score, quality of life, the results of tumor markers, cellular immune function and the cancer objective efficiency changes have been observed.Results:The main treatment-related adverse events are neurotoxicity, hand-foot syndrome, myelosuppression, nausea and vomiting, diarrhea and so on, most of which are under grade III. The incidence of neurotoxicity, hand-foot syndrome, neutropenia and anemia, nausea and vomiting in test group set significantly lower than control group. The incidence of diarrhea in test group is less than control group. The test group display more effective alleviating clinical symptoms, improving the quality of life and the physical strength, reducing the level of tumor markers, enhancing cellular immunity. Showing significant difference between the two groups (P<0.05).While comparing the cancer objective response, the cancer objective response rate in the two groups is33.33%and30.77%, the disease control rate in the two groups is81.48%and76.92%, there is no statistically significant difference between them (P>0.05).Conclusion:In this research Three-step cycle TCM therapy combined with improved XELOX chemotherapy can mitigate the chemotherapy poisonous side reaction of patients with IV period colorectal carcinoma, especially in reducing the incidence of oxaliplatin neurotoxicity, hand-foot syndrome, myelosuppression, diarrhea, nausea and vomiting, all of which well tolerated by test group. Three-step cycle TCM therapy can alleviate clinical symptoms, improve the quality of life and physical strength (Karnofsky score), reduce the level of tumor markers, enhance cellular immune function. Three-step cycle TCM therapy deserves spreading its clinical application.
