Laparoscopic Versus Open Left Lateral Segment Liver Resection: A Clinical Comparative Study

Objective: To compare the feasibility,safety and clinical efficacy of laparoscopicversus open left lateral segment liver resection. Methods: The clinical data of20patientswho received laparoscopic left lateral segment liver resection (laparoscopic group) wereretrospectively analyzed with20patients who received open left lateral segment liverresection (open group) from June2008to July2012.The operative time, the operativeblood loss, hospital stay, the postoperative complications, total hospital cost and the rate ofrecurrence were compared. Results: Laparoscopic group and open group had left lateralsegment liver resection successfully carried out. The operative time, the operative bloodloss and hospital stay in the Laparoscopic group were significant difference versus theopen group [（99.3±20.3）min vs（150.3±28.4）min，t=-6.533，P=0.000;（85.5±48.2）ml vs （146.5±48.8）ml， t=-3.978，P=0.000;（6.2±1.4）d vs （11.1±3.8）d， t=-5.411，P=0.000]. No significant difference was observed in the incidence of postoperativecomplications and total hospital cost （[2.15±0.17）×104yuan vs（.2.13±0.22）×104yuan，t=0.322，P=0.749；0vs.5.0%(1/20)，P=0.500].Among the cases,22patients with hepaticcarcinoma（12in laparoscopic group,10in open group） were followed up for3~24months,4patients（2in laparoscopic group,2in open group） had recurrent hepaticcarcinoma by CT,the rate of recurrence was16.7%(laparoscopic group) and20.0%(opengroup),it has not significant difference（P=0.632）. Conclusion: Laparoscopic left lateralsegment liver resection is feasible and safe,it has the advantages of having lesstrauma,quicker recovery for patients and significant overall efficacy when compared withopen left lateral segment liver resection.
