Objective: In the special parts(imaging stones in the left sagittal portal vein and left hepatic vein(or)proximal to the right edge)Comparative Study of the effect of left external lobectomy and laparoscopic left hepatectomy of biliary stones underwent laparoscopic left liver(Imaging of stones in the left sagittal portal vein and(or)near the right edge of the proximal left hepatic vein).Methods: Retrospective analysis of the clinical data of 60 cases from 2011 01 to2016 01 months because of the special parts of the left intrahepatic bile duct stones underwent surgery in the Second Affiliated Hospital of Nanchang University of patients,which will be 29 cases of laparoscopic left lateral lobe resection were set to the left lateral lobe resection group.Methods 31 patients undergoing laparoscopic left hepatectomy were divided into two groups.The postoperative complications,residual stone rate,recurrence rate and postoperative curative effect were compared between the left lobectomy group and the left hemi hepatectomy group.Results: Two groups of preoperative Child-Plug grade a liver function,ICG R15<10%,in the two surgical methods were used to cut the left hepatic vein and liver parenchyma.Compared with the latter(P<0.005),the amount of intraoperative blood loss and operation time were significantly higher in the left semi hepatectomy group and the left external lobe resection group(two).The recurrence rate of the left external lobe group was 13.8%,and the recurrence rate of the left half stone was about 0%.The residual rate of stone in the left lobe resection group was 17.2%,and that in the left hepatectomy group was about 0%.The incidence of postoperative complications was 27.5% in the left lateral lobectomy group,and the incidence of postoperative complications in patients with left hepatectomy was about 29%.The long-term curative effect of left hepatectomy group and left external lobectomy group was 93.3% and 73.3% respectively.Conclusion: For imaging in the left sagittal portal vein and left hepatic vein(or)proximal to the right margin near the borderline region of the left hepatic bile ductstones,laparoscopic left hepatectomy is the most effective and most thorough;...
