A Comparative Study Of Retroperitoneal Laparoscopic And Open Adrenalectomy

Objective: To evaluate and compare posterior laparoscopic adrenalectomy with open adrenalectomy.Method: The collected 47 cases of adrenalectomy from March 2006 to December 2007 in the second affiliated hospital of Nanchang University were analyzed, The 26 cases of retroperitoneal laparoscopic adrenalectomy and the 21 cases of open adrenalectomy wered classified Group A and Group B. The differences in operation time,average bleeding volume during the operation and recovery time of GI function,hospitalization stay after operation, influence on hepatic functions, renal functions and myocardial enzymogram, these date of the two procedures were compared with statistical analysis.Result:The mean operating time:group A was 3.3 min shortter than group B;the mean blood loss : group A was less diminished by 145.5 ml than group B; the mean revovery time of G1 function: group A was 1.2 day shortter than group B respectively;the hospitalization stay after operation: group A was 3.7 day shortter than group B; the consequence of biochemistry: As compared with preoperative measures,levels of hepatic functions, renal functions, and myocardial enzymogram in group A were increased at various degrees on the first day after operation, and recovered to the preoperative levels on the third day after operation, without significant difference of these parameters in changes during the peri—operative period(P>0.05), The consequence of biochemistry in the group B were constantly without significant difference of these parameters in changes during the peri—operative period.Conclusions: Carbon dioxide pneumoperitoneum during retroperitoneoscopy has no obvious influence on hepatic functions, renal functions and myocardial enzymogram.It may not cause significant damage to patients with normal heart, hepatic, and renal functions. Compared with open adrenalectomy, posterior laparoscopic adrenalectomy has the fortes: the minute injure,the less blood loss and the shortter revovery time of G1 function,the shortter time of hospitalization stay after operation ,It was the first selection of the operation on innocuousness adrenal gland tumor。...
