Objective:Lung cancer is the first high incidence of malignant tumors in the world. The peak of morbidity and mortality has never fallen. According to the statistics, the number of new cases of lung cancer in2008is about522,000.The number of deaths due to lung cancer is about453,000.The5-year survival rate is around16%. Till now, there is no way to completely cure this disease. In order to overcome it, new medical research is like an endless stream. Both clinical research and basic research of lung cancer are unable to get rid of the data mining techniques of lung cancer cases. It is necessary to deal well with the rich data resources of existing lung cancer cases and discover the the favorable data information. In the age of the vigorous development of information technology, all kinds of information management technology emerge in endlessly, the database not only become a huge amounts of data storage and management of the warehouse, and can efficiently organize and analyze data in order to meet the needs of different users. Our hospital has the rich resources of lung cancer cases, for effective and efficient use of these resources to adequately satisfy our lung cancer epidemiology investigation, clinical or basic research and translational research and realize data sharing, this study adopted a new generation of B/S (browser/server) mode database software architecture to achieve the sharing of lung cancer cases data resources. The purpose is to build a database of our online lung cancer cases.Methods:On the basis of carefully research about the process of diagnosis and treatment of lung cancer in our hospital, four lung cancer cases data resources function modes are specially investigated, which are general clinical resources table, clinical assessment resources table, serum data template, tissue speimen template. Their relations were mapped. By iterative analysis for the users’needs, a complete relational database schema was finlished in the light of the relational database design principles.On this basis, combined with functional requirements in the system, we design four modules which are data query, analysis, data management and system maintenance to meet the system requirements of different users, taking into account the expansion needs of the system.The online lung cancer cases database management system adopts B/S (browser/server) mode database software architecture, based on the openness and the research nature of its. The core software of PHP+Apache+MySQL was served as the core procedural framework for the lung cancer cases database information management system to design and publish. PHP is a server-side HTML embedded computer scripting language. Apache is a Web server. MySQL is a database based on SQL language. All of us are open source and free computer softwares, which develop and operate in cross-platform environment such as Linux, Windows and other variety of operating systems. For the convenience of easy-to-use and post-maintenance, we used Windows as the operating system platform. Ultimately, the platform WAMP (Windows+PHP+Apache+MySQL) is selected to use in the lung cancer cases database computer management system.About1,055cases which have been performed surgery in our hospital for lung cancer from January2000to December2009were entered in the database.All of them were diagnosed in the light of pathological criteria.According to the2004WHO histological classification, the histologic types of lung cancer were divided. Entering data and extraction of information about TNM staging of lung cancer from the database is in accordance with the7th lung cancer TNM classification and staging system announced by the Union for International Cancer Control(UICC) in2009.Results:The large comprehensive lung cancer cases database we have established include rich clinical resources, the reasonable data structure and the function of tissue bank. The database relies on efficient computer management system. There are four modules in it, including data query, data analysis, data management and system maintenance. The methods of the application of each module are simple and smooth, fast and easy to operate and manage. It overall responds the important role of function in the incidence, diagnosis, treatment effect etc. The data form is varied and all kinds of the lung cancer information transform dynamicly over time. Running the database, a dynamic web page presents in front of visitors. We synthesize and analyze the data of lung cancer cases from seven aspects, which are basic information management, laboratory diagnosis information management, pathological diagnosis information management, diagnostic imaging information management, clinical therapeutics information management, tumor specimen information management, clinical assessment information management. Through the lung cancer cases database system designed by PHP+Apache+MySQL core software, we can realize the data import and export capability, and expand network data sharing.We need to statistically analyze the data logged in it to determine the initial operation status of the online lung cancer cases database, and summarize data to understand the current situation of lung cancer epidemiology in our hospital, such as the constituent ratio of lung cancer, the clinicopathological characteristics, survival curve, the prognosis and so on.Conclusions:The lung cancer cases database is comprehensive and efficient, professional and scalable.and they have a wide range of the application.Not only does the database help medical staff to manag and query information ralated to lung cancer, but also it contributes to the analysis and data mining for medical researchers. At the same time the rapid development of information technology makes the vast amounts of information data stored safely, easy to search, through the statistical analysis of data could be used to mine favorable information resources, providing the basis for further effective clinical translational research. Recently there are a series of tumor databases in China, but few are associated with lung cancer directly. The establishment of the lung cancer cases database can make up for the gap.We applied a new generation of database technology to the lung cancer cases database.The database we designed and released can organize and store lung cancer datum safely. It can deal with the datum more efficiently and accurately. Under strict protection of privacy in patients with lung cancer it can also be further through the network technology to realize data sharing, reasonable development and utilization of valuable limited data resources. With the progress of clinical research related to lung cancer, the lung cancer cases database can be easy to increase or decrease the items of the data table in the assistance of computer software engineer. Is is not confined to the transverse comparison of the lung cancer incidence, diagnosis, treatment, the dynamic changes of data is also recorded over time in the background database, so that the HIS system in our hospital is more dynamic. The lung cancer cases database is not only applied to the epidemiological investigation, the disease characteristics and diagnosis, treatment, statistics, etc. In addition, it has the functions of blood specimen and tissue bank. The basic experimental research and clinical datum were closely integrated into the database. Under different exposure conditions, we can achieve statistics resources f disease remission rate, disease control rate, and the difference of toxicity of anticancer drugs. With the continuous operation of the database, we can finish the follow-up, the median survival time and overall survival time. Their results are more meaningful and guided. Construction of the database is still at the first stage. There may be a lot of problems during the operation. We need the problem of consider network security and confidentiality of medical information. With the wide use of lung cancer cases database and improving continuously, its role and influence will be more important and necessary.
