Objective:1.To investigate epidemiological characteristic and discuss relative riskfactors of zygomatic complex fractures.2. To appraise postoperative therapy of ZCFfractures, the report is intended to reveal the risk factors of prognosis of ZCF fractures.3.To explore the operative approach,the standards of reduction methods and the bestway to fix various of ZCF,Which can offer impersonality bases for diagnosing ZCFfractures.Materials and methods:1. One hundred and forty-eight patients who were admitted to the Department ofOral and Maxillofacial surgery of First Affiliated Hospital Dalian Medical Universityand underwent surgery to treat ZCF fractures from September1999to September2008were retrospectively evaluated. To investigate epidemiological characteristic of148patients with ZCF, the data of sex, age, occupation, injury causes were analyzedstatistically.2. To investigate associated systemic injuries, fracture pattern, surgical time,surgical approaches and prognosis were analyzed in48operated cases. The report isintended to reveal the best corresponding clinical treatment the risk factors of prognosisof ZCF with The SPSS16.0statistics tools for analysis and x2test.Results:1.The finding of the sample with148cases shows the minimum age was9years,the maximum age was61years, the mean age of the sample was37years, and theproportion of20-40years was the topmost (50.7%). The sex ratio4.48:1, and maleswere predominant (81.8%). The proportion of the traffic accidents was the topmostcause of ZCF, followed by fights injury accounted for16.2%, fall injury12.8%, Work-related injuries8.8%, sports injury6.7%. The incidence rate of brain damage was the topmost (35.71%) among the associated injuries with ZCF.ZCF happened all over theyear, the occurrence rates in July August September was topmost (46%).2. The sample with48cases underwent operated shows the number of the patientswere operating in two weeks was36accounted for75.0%. To appraise postoperativetherapy of ZCF were operated after three months, Efficacy grade Ⅰ,32cases (66.7%);The operative approach is more likely to be buccal sulcus approach, in37(77.1%) andthe infraorbital rim approach, in24(50.0%); For RIF status,4patients (8.3%) received1area of fixation,13(27.0%) received2areas of fixation,21(43.8%) received3pointsof fixation, and8were treated with4points of fixation. The total of pure titanium plate(153), zygmaticofrontal suture(38), maxillary buttress(39), infraorbital rim(36), fracturefragments(37).Conclusion:1. Traffic accidents are the main cause factors of ZCF. The main population of theZCF onset is20-40year-old young men. The most common associated injuries of ZCFis to the brain injury.2. The surgery can be an effective treatment to the ZCF., and it can remove thelimitation of mouth opening, occlusal disorders, diplopia, nerve dysfunction, and restorethe symmetry of the face. Type of fracture, associated injuries, as well as the timing ofsurgery affect the ZCF postoperative efficacy. For RIF status focus on zygmaticofrontalsuture, zygomatic maxillary pillar, the inferior orbital rim, the broken ends of thefracture. Two-point and three-point fixture can maintain the stability of fracture.
