In recent years,accompanying the development of transportation,cases of maxillofacial trauma increase and cause serious injuries which due to the traffic crash.For instance,the front seats driver's and passenger's face are slammed into the steering wheel or dashboard in a crash.In addition,Sports accidents and industrial accidents on construction sites are one of the causes of more such fractures.The causes also include accidental falls and conflicts in daily life,all of them have impact on the side face,resulting in the injury of zygomatic arch complex.In the past few years,it was widely used in clinical practice to treat the zygomatic arch fracture by performing Semi-coronal incision.This method has the merits of distinct exposure,and more,the scar is not obvious for the incision position is hidden in the hairline.However,due to the large number of anatomical layers involved,it is more complicated,so there will be relatively more complications.In recent years,semi-coronal incision has been gradually replaced by local small incision.According to the type and position of fracture,eyebrow bow lateral incision,eyelid margin incision,zygomatic arch under a crosscutting(horizontal),zygomatic arch under a vertical incision(vertical),vestibular sulcus incision in the mouth,maxillary sinus approach or the original wound,etc.,are chose separately.Objective : to analyze and compare the curative effect of the surgical treatment of zygomatic arch fractures with horizontal and vertical small incision,and evaluate its functional and aesthetic results and related complications.Methods :Clinical observation was performed on 92 patients who undergone facial small incision on zygomatic arch fracture in Oral and Maxillofacial Surgery of Yijishan Hospital,Wuhu,from January 2016 to August 2017.The horizontal and vertical small incisions of the zygomatic arch were selected for surgical treatment.Among them,there were 52 cases of horizontal incision of the zygomatic arch and 40 cases of vertical incision of the zygomatic arch.The efficacy of postoperative wound healing,functional stability,appearance,postoperative complications,preoperative antibiotic use and patient satisfaction were evaluated and compared.In addition,the recovery of 6 months after and 3 months after surgery was compared.Results: among the 92 patients of zygomatic fractures,the efficacy of postoperative wound healing,functional stability,appearance,postoperative complications and patient satisfaction were evaluated and compared.Statistical methods were used for data analysis,and the results show that the vertical small incision has advantages on the number of days in hospital,degree of improvement of surgical opening,the small incision scar and patient satisfaction,and the horizontal small incision has advantages on the surgical field exposure,reduction of facial nerve damage,reduce eyelid closure completely.Age difference,the difference of some complications(subcutaneous hematoma,infection of incision,pain of surgical opening,degree of improvement of surgical opening in preoperative antibiotic use and the complications of recovery of postoperative 3 months,6 months)in the two different small incision scheme,all p >0.05,had no statistical difference.Conclusion:among the local small incision in the treatment of zygomatic arch fracture reduction and internal fixation,horizontal and vertical small incision both have their respective advantages and disadvantages and adaptive cases,are suitable for wide application.In clinical practice,the choice of surgical options can be considered according to the patient's specific situation.
