[Objective]: By comparing blood loss, postoperative efficacy and kneeswelling degree between half time tourniquet and full time tourniqueduring total knee arthroplasty, we tried to find a better way of pneumatictourniquets in clinical practice.[Methods]: A prospective clinical study was taken46cases of patientsin our department underwent unilateral total knee arthroplasty at thefirst time frome January2011to July2012. All the patients wererandomized divided into half time tourniquet group (treatment group) andfull time tournique group (control group),23cases in each group. Thetotal perioperative blood loss, operative time, tourniquet time,postoperative knee swelling degree and HSS score were measured.Statistical analysis was taken to compare two methods’ advantages anddisadvantages.[Results]:1,Perioperative blood loss in treatment group wassignificantly less than the control group, mainly attribute to the hiddenblood loss and postoperative drainage less than the control group.2, Tourniquet time of treatment group was significantly less thanthe control group, while the total operation time of two groups showedno statistically difference.3, Compared to the treatment group, the thigh swelling degrees atthe first and third day after operation were more serious in control group.However, at7thday after operation, there was no statistically betweentwo groups. No significant difference between two groups on the comparisonof the degree of swelling in the calf.4, At7thafter operation, HSS score in treatment group was higherthan that in the control group, the difference was statisticallysignificant, while at third month after operation, the HSS score betweentwo groups showed no statistically difference. [Conclusions]: During total knee replacement surgery, using half timetourniquet does not increase the operation time, but significantly reducethe total perioperative blood loss. Compare to full time tournique, usinghalf time tourniquet benefit to the early postoperative functionalrecovery.
