Objective To compare and analyze the different pneumatic tourniquet application modes of operation time, peri-operative blood loss, postoperative complications, recovery of joint function, proper use of tourniquet provide reference in total knee arthroplasty(TKA) surgery.Methods February March 2014 to 2015, prospective choose to accept the unilateral total knee arthroplasty in patients with osteoarthritis in the First Affiliated Hospital of Anhui Medical University, were randomly divided into three groups, the whole group(30 cases), the first half of the group(30 cases) and the second half group(30 cases). Record groups operation time, tourniquet time, peri-operative bleeding volume, blood transfusion, limb swelling, VAS score, range of motion(ROM), HSS score, postoperative complications, postoperative hospital stays.Results The operation time of the three groups had no significant difference, but the tourniquet application time: the second half less than first half of the group and the whole group, the difference was significant. Three groups of peri-operative total blood volume and blood transfusion cases no significant difference in the number and hidden blood loss: the second half less than the first half of the group and the whole group, there was statistically significant difference. The second half of postoperative complication rate than the other two groups low, 90° of knee flexion required time than the other two groups of short, the difference is significant; limb swelling degree of patients after the first day was no different. The 3 days reached the peak, the difference is statistically significant.Conclusion Osteotomy after tourniquet application can significantly shortened tourniquet application time, reduce the hidden blood loss, reduce limb swelling, and reduce the incidence of postoperative complications, beneficial to the recovery of joint function in the early stage.
