Objective:To explore the effect of tourniquet application on the clinical outcome of total knee arthroplasty,and provide a valuable clinical reference for the application of tourniquet in TKA.Methods:Draw a table,numbered 1-60,using the EXCEL RAND program to randomly generate a T group(Use tourniquet group),and a N group(no tourniquet group).Strictly according to the inclusion criteria,Observed 60 cases of patients undergoing total knee arthroplasty in the Department of Bone Branch from the First Central Hospital of Baoding from December 2017 to June 2018.60 cases were sorted according to the table to determine whether the tourniquet was used or not.Follow up the case and collect data,Calculate the amount of blood loss during surgery,Calculate the total blood loss 5 days after operation by routine blood test,height and weight.Statistical operation time,postoperative pain(VAS score)(6 h,1d,3d,1 and 2 weeks after surgery),The range of motion of knee joint(preoperative,surgical day,and 1 d,3 d,5 d,1 and 2 weeks after surgery),Incidence of DVT within 2 weeks after operation.contrast the difference between two groups.Results:On the 1st day and 3rd day after operation,the rest and motor pain scores of T group(tourniquet group)were higher than those of N group(non-tourniquet group);The operation time of the T group was shorter than the average operation time of the N group;The amount of intraoperative blood loss and total bleeding volume within 5 days after operation were significantly lower in group T than in group N;On the day of surgery and on the 1st postoperative day,the active range of knee joints in group T was smaller than that in group N;There was no significant difference in the incidence of DVT between the two groups.Conclusion:In total knee arthroplasty,the use of tourniquet reduces intraoperative and total blood loss and shortens the operation time,but it will lead to early postoperative pain and reduce the range of knee joint activity.Whether tourniquet is used or not has no obvious influence on the incidence of DVT.In short,the use of tourniquets has an adverse effect on early pain and active knee activity,but the effect disappears after 1 week.And it can reduce bleeding and shorten the operation time and do not increase the incidence of DVT,so the tourniquet can be routinely used in total knee arthroplasty.
