Medical treatment forms are important tools of information collection and daily management for the hospital, which are consist of reports, nursing labels and specialized examination records. Through developing a medical treatment special electronic form system, which includes form design, information collection and data management, the problems which exist in data archiving, query and sharing of traditional paper forms can be solved. This is of important value for realizing electronization of medical forms and improving working efficiency of the hospital.Based on development process and related technologies of computer application system, combining with the functional requirements and business processes of the medical form system, the overall scheme of electronic medical form system is designed by applying various technologies, such as.NET, database, Web Services, XML and so on. With the analysis and comparison of the two typical system structures, from the demand analysis and user features of the system, a mixed system structure which combines C/S with B/S based on the Web Services is designed for this system.Modular design and development is used in the medical form system, which is consist of four functional modules. In the module of form designer, a WYSIWYG and graphical form designer is designed and realized with object-oriented method. Kinds of form styles can be designed by the mouse dragging simply with the form designer. The form template management module achieves the adding, design and publishing functions of a form template. The function of offline input for form data is completed with the help of XML in form data management module, and the image data interaction between image editing subform and parent form is achieved by subform’s event. In the members’ check data management module, the diagnosis mechanism for check results is built using JavaScript technique which includes abnormal judgment and reminding when check data entry, and the interactive trend chart of check data is drawn by Fusioncharts.At present, the developing of the system has been completed. All functions have achieved the design requirement generally after trial run. This shows that the overall plan of the system is feasible, and the method of problem solving is correct.
