ObjectiveUnderstand the extremely elderly patients with hearing impaired with hearing aids, hearing aid effect the status quo, matching hearing aids in patients with presbycusis, the hearing aid daily life satisfaction questionnaire (the Satisfaction with Amplification in Daily life SADL) and hearing aids, the effect of international entry (the international outcome items for hearing aids, the IOI-HA) and the quality of life of older persons questionnaire (WHOQOL-BREF on scale) fill out three questionnaires, to assess the use of effects and targeted to solve the problems encountered in its use and elderly hearing-impaired patients with hearing aids led by ENT specialist nurses to help strengthen health education, to explore the nurse-led strengthen health education for the elderly hearing-impaired patients with hearing aids to listen to the effect. So that the hearing aid to better services for the elderly hearing-impaired patients, improve the quality of life of older persons.MethodSelect March2011to2011, First Affiliated Hospital of Shanxi Medical University, ENT treatment in elderly patients with hearing impairment, before matching hearing aids, these age-related hearing impairment in patients with pure tone test, to obtain the pure-tone hearing curve. Diagnosed in elderly hearing-impaired, and pure tone audiometry manifested bilateral sensorineural hearing loss and successfully wear hearing aids patients, patients wear time has to be divided into single and double the30cases, a single intervention group, double, for the control group,60cases of control group patients only accept the guidance of the routine with a hearing aid, the intervention group patients on the basis of general guidelines, and they receive intensive health education provided by the ENT specialist nurses, including health lectures, one-on-one personalized education and health promotion material released. Inform these patients with presbycusis return visit to the hospital for three weeks after hearing aid fitting and March. Each return visit in the process, to enable patients to fill in the way of face to face inquiries SADL and the IOI-HA questionnaire. Finally, the questionnaire results of these patients with presbycusis.Results1. The baseline results show:the study hearing aid wearing effect score was2.70±0.30; patients with hearing aids daily life satisfaction score was2.79±0.30; elderly quality of life questionnaire (WHOQOL-BREF on scale) to measure the overall quality of life in patients with score of11.94±1.20. Multiple regression:the study of the IOI-HA and SADL scores and age, educational level and hearing damage. 2. Paired t test, control group and intervention group, respectively, after the IOI-HA, SADL WHOQOL-BRBF, score results:(1) before intervention in the control group, after the IOI-HA, SADL, and the WHOQOL-BRBF score slightly increased, the difference was statistically significant (P<0.05);(2) the intervention group before the IOI-HA and SADL score higher than those before the intervention to increase, and the difference was statistically significant (P <0.05).3. Respectively, on the the SADL four dimensions of the control group and intervention group before and after scores were compared using a paired t test results showed that the four scores increased scores after the intervention, the intervention group before and after four dimensions score after the intervention scores have increased, and the differences were statistically significant (p<0.001).4. Repeated measures ANOVA analysis showed that the time main effect of the IOI-HA (1), SADL WHOQOL-BRBF,, the total score was statistically significant (p<0.05), does not consider the factors of the intervention, patients with the IOI-H, SADL and WHOQOL-BRBF Total points changes over time; intervention main effect of the IOI-HA, SADL, and the WHOQOL-BRBF the total score was statistically significant (p<0.05), without considering the time factor, the different levels of intervention, patients with the IOI-HA, SADL WHOQOL-BRBF, total difference; the IOI-HA score, SADL score and the WHOQOL the-BRBF Total time factors and intervention levels there was an interaction (p<0.05). Using paired t test, respectively compared to the control group and intervention group before and after the WHOQOL-BRBF score, the results displayed in the control group before the intervention, the WHOQOL-BRBF score increased slightly, but the difference was not statistically significant (p>0.05The); before the intervention group, after the WHOQOL-BRBF score increased, and the difference was statistically significant (p<0.05).6. Paired t test, respectively, scores were compared to the control group and intervention group, after the WHOQOL-BRBF four areas, the results show:(1) score of four areas of control group scores after the intervention has increased, but psychological domain score difference was statistically significant (p<0.05), physical, social relations, the field of environment before and after the intervention, the difference was not statistically significant (p>0.05).(2) The intervention group, after four areas scores after the intervention scores have increased, in addition to the physical domain was no significant difference (p>0.05), the other three areas of differences were statistically significant. ConclusionElderly hearing-impaired patients with hearing aids, hearing aids are not effective and widespread, serious harm. And analysis by filling out the questionnaire of the SADL and the IOI-HA two hearing, ENT nurse led intensive health education to improve elderly patients with hearing impairment with the theme after hearing aid effect on hearing aid satisfaction and benefit improved significantly.By filling in the elderly quality of life questionnaire (WHOL-BREF scale), led by ENT nurses strengthen health education at the same time also improve their quality of life, elderly hearing-impaired patients with hearing aids after the implementation of the comprehensive nursing intervention can help patients with comprehensive rehabilitation.
