ObjectivesThrough a systematic and scientific scale construction procedure,we construct a self-evaluation scale of health education competence for nursing staff and conduct reliability and validity tests to provide guidance and guidance for scientific,comprehensive and objective evaluation of health education competency of nursing staff.in accordance with.At the same time,on this basis,we explore the relationship between nursing staff’s health education competency,self-efficacy,and organizational support,and provide support for later intervention and improvement of nursing staff’s health education competency.MethodsThe first stage: Through the literature review method,focus group interview method,and Delphi method,preliminary indicators of all levels of the scale(forecast version)are established.And using the scale and the cluster sampling method to conduct a pre-survey of 727 clinical nurses in a tertiary A hospital in Anhui Province from March to June 2019(recovered 626 valid questionnaires),after project analysis and the first exploratory survey Factor analysis forms the tested version of the scale.The second stage: Using the cluster sampling method,from July to October 2019,2,036 clinical nurses in 4 third-level first class hospitals in Anhui Province were formally administered(1,861 valid questionnaires were recovered),and the second exploratory factor analysis and verification Factor analysis and internal consistency reliability test form the application version of the scale.The third stage: Using cross-sectional surveys and convenient sampling methods,10,239 nursing staff from 23 first-level hospitals,56 second-level hospitals,and 54third-level hospitals in Anhui Province were surveyed from July to September in 2020.The study used the Social-demographic Questionnaire,the Self-efficacy scale,the Organizational Support Perception Scale,and the Health Education Competency Self-evaluation Scale for Nursing Staff to collect the data.A total of 10,239 questionnaires were issued,9,182 questionnaires were effectively returned,and the effective response rate was 89.68%.And we used independent sample t test,Pearson correlation analysis,and multiple linear regression methods for data analysis.ResultsThe first stage: Through literature review method,focus group method,Delphi method and pre-survey,a test version of the self-evaluation scale of health education competence for nursing staff with knowledge and experience,skills and abilities,and comprehensive qualities as the main framework was formed,including there are 3first-level indicators,17 second-level indicators,and 90 third-level indicators.Among them,the two rounds of Delphi method experts’ positive coefficients are both 100%.The expert authority coefficient is 0.855,and the expert coordination coefficient is0.611 and 0.635 respectively(P<0.01).At the same time,the pre-investigation did not delete any items,but only adjusted the attribution of individual items and renamed the factor names.Thus,the measurement version of the scale includes 3 first-level indicators,10 second-level indicators,and 90 third-level indicators.The next step is to study the reliability and validity of the scale.The second stage: After the formal implementation of the test,an application version of the self-evaluation scale of health education competence for nursing staff with knowledge and experience,skills and abilities,and comprehensive qualities as the main framework has been formed,including 3 first-level indicators,17 second-level indicators and 89 three-level indicators.Among them,confirmatory factor analysis shows that the three sub-scales have good structural validity.The fitting indices of knowledge and experience,skill and ability,and comprehensive quality are respectively:the ratio of chi-square degrees of freedom(X2/df)is 2.169,2.829,2.836,comparative fit index(CFI)were 0.999,0.971,0.991,asymptotic residual square root(RMSEA)were 0.035,0.044,0.044,normative index(NFI)were 0.998,0.956,0.986,The goodness of fit index(GFI)was 0.993,0.857,and 0.957,respectively.The total Cronbach’s α coefficient of the scale(applied version)is 0.977;the Cronbach’s αcoefficients of the three sub-scales of knowledge and experience,skills and ability,and comprehensive quality are 0.965,0.983,and 0.934,respectively;the half-reliability and CVI of the scale are 0.811,respectively,0.943.The third stage: The average scores of nurses’ self-efficacy and organizational support were(29.45 ± 6.16)and(48.49 ± 11.26),respectively.The health education competency score of nursing staff was(78.08±11.38),which was in the middle level.At the same time,there are significant differences in health education competency in age,working years and professional title(P <0.05).Health education competency was significantly positively correlated with self-efficacy and organizational support(P<0.001).There was a significant positive correlation between self-efficacy and organizational support(P<0.001).Organizational support has a direct and positive predictive effect on health education competency.Self-efficacy plays a mediating role between organizational support and health education competency.The mediation effect is 0.336,accounting for 51.45% of the total effect.ConclusionThis study finally formed a self-assessment scale for health education competency of nursing staff,including 3 sub-scales and 89 items.Among them,the "Knowledge and Experience" sub-scale includes 2 dimensions("Medical Knowledge" and "Experience"),with a total of 8 items;"Skills and Ability" includes 5 dimensions("evaluation","planning,implementation and evaluation","scientific research and innovation","communication","leadership and management"),a total of 59 items;"Comprehensive quality" includes 3 dimensions("social role","personal characteristics and motivation","professional loyalty"),a total of 22 items.And after strict reliability and validity test,it has good reliability and validity.At the same time,studies have found that self-efficacy plays a mediating role between organizational support and health education competency.
