| The new type of rural cooperative medical care system can be defined as a systemorganized, guided and supported by the government and farmers can voluntarily chooseto participate in it or not. It is financed by multi-party, includes the individual, collectiveand government, which mainly focuses on serious illness co-ordination, based on aprinciple of farmer mutual aid union. This system has been run for a decade since2003,has basically achieved the goal of full coverage of rural residents, effectively eliminatethe burden of health care costs, reduce the phenomenon of illness causing poverty andreturning to poverty due to illness. However, the system itself has some drawbackswhich create many problems during operation, such as low level of fund security,difficulty to erase adverse selection and poor results of protection. The theory of justice,institutional change and public goods theory are the theoretical basis of this paper. Firstthe definition of the new type of rural cooperative medical care system and basic socialmedical insurance are given, then we systematically analysis the achievements andshortages during the operation of this system. Furthermore, the basic social medicalinsurance mode of Dongguan was used as case study to illustrate a unified social basicmedical insurance should be build, which comprises the new type of rural cooperativemedical care system as well as the basic medical insurance of urban workers anddwellers to assure everyone can benefit from basic medical insurance. |
