Research BackgroundAt present, people all around the world are suffering from infectious diseases or the threat of infectious diseases.With the continuous development of medical technology and unceasing enhancement of medical service, infectious disease has declined in the overall spectrum of disease and death, and has been replaced by chronic non-communicable diseases and accidental injury. The global burden of disease study in1990estimated that, in the distribution of death in developed countries and many developing countries, chronic non-communicable diseases are the leading cause of death.In developed countries, about3/4deaths are caused by cardiovascular diseases, cancer and injuries;in many developing countries, chronic non-communicable diseases has become one of the most common causes of death, and the harm done by chronic non-communicable diseases to people has far exceeded the infectious disease. Therefore, prevention and control of chronic non-communicable diseases has gradually become the main task of disease prevention and control department at all levels. Previous death retrospective survey results of Shandong province show that, through1985-1989and1990-1992, the mortality rates of malignant tumor are115.64/100,000and118.32/100,000respectively, ranking the third in all causes of death.High cancer incidence age is between40-65years old, which seriously affected the health of labor force and led to great social and economic burden.Along with the increase of urban population year by year, environment pollution, smoking, unhealthy life style and diet are resulting in some of the most common cancer morbidity and mortality significantly. In the face of the threat of disease and health problems, how to allocate the limited health resources and determine the key of disease prevention scientifically and reasonably is a common task facing all governments in their health-related decision-making.Therefore, a comprehensive analysis and grasp of the characteristics of death and disease burden indicators of local residents is of great significance for disease prevention and treatment in improving the quality of people’s life.Research ObjectivesTo understand and grasp the residents’total death level, death ages and gender distribution.To master the sequence of all-cause death and death situations, the composition of death cause, ages, gender and geological distribution characteristics of the first three diseases among local people.To provide governments at all levels with the scientific basis of disease prevention and control and health care decision-making in the regions, and offer scientific basis and the baseline data for research on the circulatory system disease, malignant tumor, and accident prevention and control measures in the future.Research methodsThis study, using census method, is a retrospective survey of death causes of all residents in Tai’an city in2010. In descriptive and cross-sectional method, the study, in accordance with "Tai’an City death registration information network report specification", carries out the investigation in death and death cause, audits death toll, collects current population and birth population data in Tai’an city in2010, sorts out and analyses these statistics with death cause surveillance data cleaning and analysis software.The results of the studyThe research showed, in Tai’an city in2010,0years life expectancy is76.35years old, with male73.25and female79.65years (the national average life expectancy is73years old in2009). The death surveillance reporting system suggested that the death toll of local residents in2010was30512, and the total mortality rate was548.87/0.1million, including17794male(632.56/0.1million) and12718female cases(463.26/0.1million). In2010, there were54409live births and193infant deaths in Tai’an city. The infant mortality rate was354.51/0.1million. In2010, the top ten death causes of local residents were malignant tumor, heart disease, cerebrovascular disease, injury, respiratory system diseases, circulatory system disease, nervous system disease, digestive system disease, endocrine, nutritional and metabolic disease, infectious disease and parasites. The total mortality rate of local residents died of malignant tumors in2010was138.53/0.1million, with male mortality rate185.39/0.1million and female mortality rate90.98/0.1million.The suggestion of the studyIn2010, the main cause of death for both males and females in Tai’an city was chronic non-infectious diseases, accounting for more than80%of all death causes, with females’higher proportion in chronic diseases than males and males’ higher percentage in injury death than females. The top ten malignant tumor related diseases killing local residents in2010are lung cancer, esophageal cancer, gastric cancer, liver cancer, colorectal cancer, leukemia, breast cancer, brain cancer, pancreatic cancer, bone and articular cartilage malignant tumor related diseases.Prevention and control of chronic non-communicable diseases is a systematic project which needs the participation of government and whole society. Special efforts should be made, especially during current period of economic transformation and sudden change in people’s way of life, in health education and behavioral intervention, in prompting people to change their life style and quit bad behavior in order to lower residents’morbidity and mortality caused by chronic diseases, malignant tumor, cerebrovascular disease, etc..
