BackgroundThe change of mortality rate and cause of death are important indicators reflects the health condition of resident, which is also the scientific basis for establishing health policies, evaluating the quality and effect of the health service, in addition, which is an important content of studying the dynamic of natural population activities. Dynamical collection and system analysis of the death information make a significant contribution to formulate the policies of population and health correctly, allocate of health resources reasonably, prevention and control the priority diseases, protect the people health and promote the sustainable socioeconomic development.ObjectiveTo find out the prevalence status of mortality, death composition and its variation with the data from the National Disease Surveillance Spot System during2010~2011for residents in Zibo city of Shandong province, and use the indicator of Years of life lost (YLLs) to perform preliminary study on the burden of disease (BOD) status, determine the major diseases affecting the health for the residents of Zibo City, consequently provide scientific evidence for Government and health administrative departments to determine the priority issues of health of our city, adjust the strategies of disease control and prevention, allocate of health resources reasonably.MethodsThis study is a descriptive study but also a health economics study. The statistic analysis of this study were based on the data from the National Disease Surveillance Spot System during2006~2007for residents in Zibo city of Shandong province, the indicators of mortality and Constituent ratio were used to describe the prevalence status of mortality level, the mortality of different regions, different populations and different disease were also compared. The indicator of Years of life lost (YLLs) was used to perform the status of the burden of disease (BOD).Results1.48537death cases were reported through the National Disease Surveillance Spot System during2010-2011for residents in Zibo city of Shandong province, among which it was27674for male and20863for female. The crude mortality rate was575.24/100,000, among which it was655.52/100,000for male and494.86/100,000for female. The crude mortality rate of female was higher than the female, male was1.325times than the female. The crude mortality rate showed "(?)"curve with the age increase, the5~age group showed the lowest mortality rate, the mortality rate of0~age group was higher than the5~age group, the mortality rate increased from the10~age group. The average life expectancy of the residents in Zibo City from2010to2011was78.18years old, among which it was76.04for male and78.18for female.2. According to the death composition, non-communicable diseases (NCD)(the first category) occupied the leading causes of death, followed by injury (the second category) and infection, maternal and nutritional deficiency diseases (the third category). The crude mortality rates of these three categories disease of female were all higher than the male. The first category disease was the leading causes of death of0age group, the second category disease was the leading causes of death of1~4age group, the5~14age group was main died in injury and infection, maternal and nutritional deficiency diseases. The mortality rate caused by the second category showed an apparent upward trend, and became the leading causes of death.3. According to the rank order of causes of death, the top five causes of death were cardiovascular disease, malignant tumors, cerebral vascular diseases, chronic respiratory disease and injuries; they were the leading cause of death of the residents in Zibo City with accounting for79.37%of the total deaths. Malignant tumors was the second rank order of causes of death of the total population, which was also the first rank order of causes of death of the male, and which is the first rank order of causes of death of the population aged from45to64. The top five causes of cancer death among population were lung, gastric cancer, hepatic carcinoma, esophageal cancer and colorectal cancer with accounting for67.12%of the total cancer deaths. The top five causes of injury deaths among population were traffic injuries, suicide, accidental poisoning, accidental injury and accidental falls.4. BOD caused by various diseases was541622years of life lost, equivalent to an annual loss of64.19YLLs per1000population. Among which it was326090YLLs for male (equivalent to an annual loss of77.2YLLs per1000population) and215532YLLs for female (equivalent to an annual loss of51.1YLLs per1000population). The BOD showed caused by various diseases an apparent upward trend.Conclusion and suggestionWith the development of economy and level of health care, the living standards of the residents in Zibo City had been increasing constantly; the pattern of death had been changing all the time, non-communicable diseases (NCD) had become the main cause of death in Zibo City, the adverse effects of NCD on residents’health and life has been increasing with every passing day, especially with the aging population degree is deepened, the NCD will inevitably make more heavy BOD in the future. Therefore, emphasis should be put on the health education of the whole population, encourage residents form healthy behaviors and lifestyles, all measures should be taken to remove the risk factors causing the NCD to improve the health level of the residents.
