Investigation And Research On The Application Of Remote ECG Monitoring Between Hospital And Community

Cardiovascular disease has become the biggest threat to human health, to explore the establishment of inspection and treatment system extends to the outside of hospitals is a new way to improve the level of prevention and treatment of cardiovascular disease. Remote ECG monitoring is a remote medical mode, it refers to the use of modern communication technology, computer technology, electronic technology, network technology, the collected ECG information is transmitted to the care center located in different places, what’s responsible for analysis and diagnosis the ECG information to guide community physicians to carry out medical activities. A remote ECG monitoring system consists of three parts of the client ECG data transmission, server-side monitoring center, the formation of an information collection-transmission-analysis-diagnosed in one complete system.In2011March, the affiliated hospital of Hangzhou normal university and three community health service centers and its affiliated site in Gongshu district in Hangzhou city pilot build remote ECG monitoring system, this article attempts to analysis of the pilot phase of the project in operation mode and organization and management process, to study the status of project implementation, and summarize the lessons, analysis of the shortcomings and problems, and put forward the improvement measures in order to be helpful for the sustainable development of the project.Through the literature research and analysis of remote ECG monitoring project conducted by the social background, combing remote ECG monitoring system development history, the application scope and the significance, analysis of the remote ECG monitoring project construction, organization management of process and operation. Application the self-designed interview outline to conduct in-depth interviews the main interests involved stakeholders-patients, community general practitioners, administrators, hospital physician, to ascertain their attitude to the project, the main stand, interests etc; some patients with application of remote ECG monitoring service for the experimental group, the patients directly to the hospital for ECG examination as control group, using the self-designed questionnaire to survey patients on the inspection fee, waiting time and the degree of satisfaction; analysis of the existing problems and their causes of project, and then proposes some corresponding improved measures and countermeasures.The process of organization and management of the project is relatively smooth:the powers and responsibilities of hospital care centers and community monitoring sites were clearly defined; since running, the monitoring center received a total of1092ECG information, center physicians provided timely diagnosis, there were434abnormal ECG information, the abnormal rate was39.74%, to find and rescue the three heart patients with critical disease; the survey found that application of remote ECG monitoring of patients with treatment time, waiting time, inspection fees were less than direct to hospital patients, higher satisfaction, and the difference was statistically significant (P<0.05); the project has also been received the affirmation of community physicians and the attention of provincial and municipal administrative department of health and the Hangzhou mainstream media, and achieved a good social reputation.The pilot project has achieved several good results, but there are also some problems to be addressed, to be guaranteed capital investment, to ensure balanced partner interests, to improve the prevention and disposal of medical malpractice, ECG information safekeeping and utilization, patient privacy protection and other aspects of the regulations; to provide more convenient monitoring instruments; to increase publicity:to motivate staff enthusiasm and other measures to promote the healthy development of the project.
