ObjectiveTo explore TCM syndrome differentiation laws of on asthenopia and analysis of its related factors in modern life through surveys of symptoms and signs of asthenopia patients.MethodsWe schemed out medical history questionnaires on the basis of literature research and expert consultation to collect the general information and TCM symptom-sign from111cases of asthenopic patients admitted to the Departmentof Ophthalmology in affiliated hospital of Chengdu University of TCM. and gave them some related examinations such as visual acuity, optometry, accommodation, recessive strabismus, convergence, intraocular pressure and etc. We put the data of quantified TCM symptom-sign (including the symptoms of tongue and pulse) into computer to establish the database after converting the above information into qualified variables parameters. SPSS17.0statistics software was adopted to analyse the general information, frequencies of TCM syndromes and TCM syndrome scores with descriptive statistics; the adjusted syndrome variables and clinical syndrome types were analyzed with K-means cluster statistics and was divided into four categories to identify the syndrome types of each category based on TCM theories and professional knowledges, and to compare them with clinical syndrome types.Results1. In111patients with asthenopia, the male accounted for27.93%while the female accounted for72.07%. We contrast their proportion in male and female and found out there was significant difference between them (P<0.01). We contrast the severity in both male and female groups, there was no significant difference between men and women (P>0.05).2. In111patients with asthenopia, the person of ametropia accounted for90.99%. 3. In111patients with asthenopia.65patients showed of mild asthenopia,36patients showed of moderate asthenopia, and10patients showed of severe asthenopia. We contrast their proportion in male and female groups, there was significant difference (P<0.01).4. In111patients with asthenopia, the most common symptoms in descending order were dry eyes, eye fatigue, orbit acid bilges, eye pain, hypopsia. blurred vision, sourness of eye. neck shoulder ache, lassitude (frequency>50%):The symptoms which had the highest TCM scores were dry eye, eye fatigue, eye pain, orbit acid-bilges, hvpopsia. blurred vision, neck shoulder ache.tinnitus. lassitude, eye itch, eye sourness, amnesia (TCM scores>200credits):The symptoms with both high score and frequency were dry eyes, eye fatigue, eye pain, orbit acid bilges, blurred vision.5. The symptoms and signs of111cases of asthenopia patients, were analyzed with K-means cluster statistics and divided into4categories, the results of syndrome differentiation suggested that the first type (yin deficiency of liver and kidney) accounted for61.26%. the second type (liver-kidney yin deficiency with qi stagnation) accounted for19.82%, the third type (liver-kidney yin deficiency with dampness) accounted for11.71%. the fourth type (liver-kidney yin deficiency with dampness and qi stagnation) accounted for7.21%, which was in accordant with clinical syndrome types.6. Among111patients with asthenopia,65showed of mild asthenopia,36showed of moderate asthenopia, and10showed of severe asthenopia, we contrasted their proportions in four categories by pare comparison, there was no significant difference between the first type and the third type (P>0.05). and between the second type and the fourth type. Except for that, there were significant differences between another pared comparisons (P<0.05).Conclusion1. The morbidity of asthenopia from high to low were mild, moderate and severe asthenopia. And female were more involved than male, however, there was no significant difference between female and male in each classifications.2. Local eye symptoms were the main manifestations of asthenopia. accompanied with whole body symptomsand correlated with psychogenic factors. The common symptoms of asthenopia from high to low were dry eyes, eye fatigue, orbital acid bilges, eye pain, blurred vision, eye sourness, neck shoulder ache, lassitude, eye itch, amnesia, temporal pain and etc.3. TCM syndrome types of asthenopia were categorized into3, which were live-kidney yin deficiency, liver-kidney yin deficiency with qi stagnation, and liver-kidney yin deficiency with dampness. Amony them, liver-kidney yin deficiency was the most common type, some syndrome types were compounded with each other.4. There were seven etiologies of asthenopia, that were eye over use, myopia, hypermetropia, presbyopia, emotion injury, dampness attack, food partiality, eye over use, myopia, hypermetropia and presbyopia were the most common etiology. The main pathogenesis was deficiency, deficiency-excess complication, and yin deficiency, which was lingered with dampness, played a major role in leading a disease. The location mainly lied on liver and kidney.
