Objective:Polycystic ovary syndrome is a kind of hyperandrogenism, which is characterized by ovulatory dysfunction and lesions, and is the main cause of menstrual disorders and infertility. The long-term complications are diabetes, cardiovascular disease, endometrial cancer and breast cancer. Many studies suggest that PCOS may originate in adolescence and continue into adulthood. The rate of women of childbearing age prevalence is5%to10%and there is no exact statistics about the incidence rate of adolescent PCOS. The clinical features of adolescence and childbearing age are similar, but adolescence are mainly characterized by menstrual disorder, obesity, hairy, acanthosis nigricans, and women of childbearing age can cause infertility and natural abortion etc. Obesity increases the risk of complications of adolescent PCOS patients after they become adults. There is no polycystic ovary syndrome in Traditional Chinese Medicine. According to the different clinical symptoms. Chinese medicine believes that the disease is related to the dysfunction of liver, spleen, and kidney, as well as phlegm-dampness and blood stasis. This study observed the clinical efficacy of Bushen Jianpi Huatan Decoction in the adolescent PCOS patients. Methods:66cases of adolescent outpatients of PCOS patients who meet the inclusion criteria for this study selected from June2010to December2011in People’s Hospital of Shiyan. The height (m), weight (kg),waist circumference (cm, abdominal circumference) of the umbilical level, hip circumference (cm, the level of sacroiliac joint, hip circumference) of all cases were measured, a detai led medical history was taken from all patients, detected the basal body temperature (BBT)by oneself, determined ovarian volume and follicle number under the abdominal ultrasonography, collected venous blood to assay serum luteinizing hormone (LH), follicle stimulating hormone (FSH), estradiol (E2), testosterone (T), fasting insulin (FINS), fasting plasma glucose (FPG), calculated body mass index (BMI), waist to hip ratio (WHR), LH/FSH, homeostasis model insulin resistance index (HOMA-IR). Randomly divided into two groups, Traditional Chinese Medicine (TCM) group (exit5cases) party28patients, western medicine group party (exit1case)32cases. TCM group were given to Bushen Jianpi Huatan Decoction for basic party to add and subtract. The treatment period of the TCM group was3menstrual cycles. The patients in the Western medicine group were given oral Diane-35every night in the fifth day in menstrual cycle or retreat21days continuously at the beginning of the fifth day after hemorrhages at a dose of1pill in fixed time. The interval between repeat administration were7days and lasted three menstrual cycles altogether. Taking metformin twice for3menstrual cycles at a dose of0.5g simultaneously. This study observed the improvements of acne, BBT and menstruation before and six months after the end of treatment in both groups. It observed the improvements of menstruation during the treatment in two groups. And it also observed the changes of WHR, BMI, E2, T, FSH, LH, LH/FSH, FPG, FINS, HOMA-IR, acne score, as well as the volume of ovary, follicle number under the abdominal ultrasonography before and after the treatment in both groups. And it compared the differences of the comprehensive effect between two groups.Results:As for clinical symptoms before treatment, the performance of the menstrual abnormal ities were28cases in the Chinese medicine group and32cases in the western medicine group respectively, appearance of acne were10cases in the Chinese medicine group and8cases in the western medicine group, and the BBT were single-phase in both groups, the difference were not statistically significant (P>0.05) between two groups. The efficacy results showed that the improvement of menstruation were23cases in the Chinese Medicine group and32cases in the western medicine group during the treatment period, the difference between two groups was statistically significant (P<0.05). Six months after treatment, the improvements of menstruation were18cases, acne improvements were6cases and BBT duplex were17cases in the Chinese traditional medicine group. The improvements of menstruation were12cases, acne improvements were3cases and BBT duplex were10cases in the Western medicine group. The differences were statistically significant in menstruation and BBT (P<0.05). The acne improvement between two groups was not statistically significant (P>0.05). BMI, T, LH, LH/FSH, FINS, WHR and HOMA-IR, acne score, as well as ovarian volume and follicle number under abdominal ultrasonography were significantly different before and after treatment in both groups (P<0.05), but the differences were not significant between two groups (P>0.05). E2, FSH and FPG were not significantly different before and after treatment in both groups (P>0.05).Conclusion:(1)Bushen Jianpi Huatan Decoction can achieve the aim to improve the sex hormone levels, reduce FINS, improve insulin resistance, improve acne and polycystic ovary conditions.(2) As for menstrual improvement during the treatment, the efficacy of western medicine group is superior to the Chinese Medicine group. Six months after the end of treatment, the therapeutic effect of the Chinese Medicine group is more effective than the western medicine group.(3)The comprehensive efficacies assessments of both groups are considerable and show no significant differences.(4) After the treatment of three menstrual cycles, drug side effects were not found. It showed that Bushen Jianpi Huatan Decoction is safe and effective.
