| Objective:Polycystic ovary syndrome(PCOS),one of the most common endocrine diseases with reproductive dysfunction and abnormal glucose metabolism affecting women of reproductive age, plays an important role in anovulatory infertility. Long-term complications of PCOS are diabetes, cardiovascular disease, endometrial cancer and breast cancer. Its important features are Persistent anovulation,androgen excess and insulin resistance. A large number of studies confirm that the insulin resistance has an close relationship with PCOS,and Nearly40%-60% of PCOS patients have insulin resistance.In the view of Traditional Chinese Medicine(TCM),PCOS classifies to "menopathy", "infertility"and "Zhengjia", Many scholars insist that pathogenesis of PCOS is a renal deficiency plus phlegmatic hygrosis and blood stasis. Clinical syndrome is divided into deficiency of kidney type, sputum dampness type and blood stasis type, etc.The research aims to evaluate the therapeutic effect of BuShenHuaTanQuyu Decoction on treating patients with PCOS and IR,and investigate the therapeutic mechanism.Methods:In the fisrt part of clinical sutdy, selecting 45 patients with PCOS and IR, Recording their mensturation,age,weihgt,height.The study assessed serum LH,FSH,E2,T,FINS,FPG and ovarian volume and follicle by Ultrasound.calculated body mass index(BMI),LH/FSH and HOMA-IR. Classify them into two groups-the treatment group(30 patients) treated with Chinese traditional medicine, the control group(15 patients) treated with Metformin.The course of treatment with BuShenHuaTanQuyu Decoction should be last 3 months. Western medicine group of patients taking metformin, a dose of 0.5 g, twice for 3 months,adding to oral Diane-35 in the fifth day in menstrual cycle, a dose of 1 pill in fixed time, once for 3 months.In the senventhday after discontinuation,repeated to medicine.Before and after the treatment, the clinical symptoms and signs, reproductive hormones, serum FINS, FPG, BMI and HOMA-IR were observed.Results:In the clinical symptoms, the Chinese traditional medicine group and western medicine group before menstruation, there were irregular menstruation 27 cases, respectively in 13 cases, the infertility 7 and 3, monophase of BBT 25 and 12.Therapeutic effects displayed that the both group could improve menstruation and BBT in Chinese traditional medicine group, after treatment, normal menstruation 18 cases and 10 cases, BBT restore duplex 19 cases and 10 cases, notable differences (P<0.05).One cases infertility in the both group were pregnant after treatment. The levels of serum T,LH. LH/FSH,FINS and HOMA-IR were lower than those of pretherapy(P<0.05),no difference was found between two groups.And the ovarian morphological observation under ultrasound, before and after the treatment, was significant difference in two groups (P<0.05).The same curative effect in the both group.Conclusion:(1) BuShenHuaTanQuyu Decoction was proved efficient and no toxic side effects in the clinical research. (2) BuShenHuaTanQuyu Decoction may treat patients with PCOS and IR not only through improving menstruation, lower testosterone, LH,precipitating ovulation, pregnancy, but also reducing serum insulin level and Improving IR, regulating the unbalanced incretion, so as to induct ovulating.(3) The total effective rate in treatment class and in control class has no significant difference between them. |
