Objective:(1)Observation of oral submucous fibrosis coexisted with oral Leukoplakia patients with clinical manifestation and pathological features to discuss the relationship between the degree of epithelial hyperplasia and the severity of fibrosis.(2)Detection of the expression and distribution of apoptosis gene Bcl-2protein in oral submucous fibrosis (OSF), oral keokoplakia (OLK), oral submucous fibrosis coexisted with oral Leukoplakia, fibrousbecome cancerous tissues to probe the role of expression of Bcl-2in the cancerization of OSF coexisted with OLK.Method:(1)89specimens of74cases of the original data were collected.(2)Morphological information included:pathology reports,biopsy and histopathological wax block. Observed the morphological characteristic in the light microscope and made detailed records.(3)Take5specimens of normal oral mucosa,10specimens of OLK,15specimens of OSF(early, middle, late of5specimens, respectively),15specimens of OSF coexisted with OLK,10specimens of OSF cancerous lesionsin patients with fresh tissue, which was paraffin-embedded. Immunohistochemistry (SP method) were used to detect the disparity of Bcl-2expression and distribution. Result:(1)OSF coexisted with OLK patients were more common in middle-aged males. Patients aged20-49were among a higher prevalence of86.48%, and they were all chewing betel nut.(2)Light microscope performance:in the89specimens of74cases of OSF with OLK patients, specimens of epithelial dysplasia were68,including31specimens of mild hyperplasia,22specimens of moderate hyperplasia,15specimens of severe hyperplasia,epithelial hyperplasia in21specimens. The performance of electronic microscope:epithelial cell gap was widened, epithelial cells were deformed,the number of mitochondria was reduced and visible edema or degeneration. Endoplasmic reticulum,basement membrane was thicker.Edema and degeneration of collagen fibers as well as glass-like change.Bcl-2protein in5specimens of normal oral mucosa epithelium was negative or very weak. In15specimens of OSF, the early increased expression,and with the development of disease,the expression gradually increased,the expression in15OSF coexisted with OLK specimens was significantly increased.10specimens of OSF cancer group and normal control group,OSF early and middle group and OLK group had significant difference(P<0.05),but with the OSF late group,OSF with OLK group was not significant(P>0.05),OSF coexisted with OLK and OLK group had significant difference(P<0.05).Conclusion:(1)More common in patients with OSF coexisted with OLK were young male patients,and they displayed the clinical symptoms and signs of both OSF and leukoplakia.(2)The extent of epithelial dysplasia and the severity of fibrosis were not related in OSF coexisted with OLK patients.(3)Bcl-2protein rate in OSF coexisted with OLK and OSF with cancerous tissue were higher than the positive expression rate of OSF and OLK, suggesting that the protein played an important role in the cancerization of OSF with OLK through the inhibition of apoptosis.
