The Clinical Prognosis Of Posterior Cervical Unilateral Open-door Expansive Laminoplasty Plus Centerpiece Plate Fixation For Cervical Spondylosis

Objective:To evaluate the clinical efficacy of posterior cervical unilateral open-door expansive laminoplasty plus Centerpiece plate fi-xation for cervical spondylosis.Methods:From March2009to July2011,25cases suffering from cervical spondylosis underwent unilateral open-door with Centerpiece fixation formed the basis of this study. All cases were treated in Qilu hospital of Shandong University. Of these cases, there were14males and11females, with a mean age of61.2years (range,34～78years). The clinical manifestations were pain and discomfort in the neck and shoulder, upper extremity numbness, pain, lower limb weakness.13cases were diagnosed with nerve root type cervical spondylosis,5cases with cervical spondylotic myelopathy, and7cases with mixed cervical spondylosis.3cases of patients with dizziness. Preoperative MRI showed three or more than three levels lesion in all cases. The neurofunction was evaluated by Japan Orthopaedic Association (JOA) score、the improvement rate and FTT (foot tapping test).Besides, the axial symptoms and the change of cervical curvature were also observed after operation.Results:All patients were followed up for8-16months(mean10.6months). The average JOA score for preoperation was8.3±2.6and14.2 ±2.0after operation. With the improvement rate of67.8%. The average preoperative FTT (foot tapping test) was22.1,while the postoperative average FTT was27.2. Imaging studies showed that there was no statistical insignificance in cervical curvature changes before and after operation. Two cases of patients occurred axial symptoms after surgery, such as neck soreness and muscle spasms.Conclusion:Unilateral open-door expansive laminoplasty Centerpiece plate fixation for cervical spondylosis is efficient in improving the symptoms, maintaining the cervical lordosis and reducing the incidence of axial symptoms. And the clinical efficacy is satisfied.
