Evaluation Of The Clinical Effect Of Posterior Cervical Unilateral Open-door Laminoplasty With Lateral Mass Screw Fixation In The Treatment Of Cervical Spinal Stenosis And Cervical Instability

ObjectiveObserve the clinical effect of posterior cervical unilateral open-door laminoplasty with lateral mass screw fixation in the treatment of cervical spinal stenosis and cervical instability.MethodsRetrospective analyze the 15 patients who were treated by posterior cervical unilateral open-door laminoplasty with lateral mass screw fixation from May 2012 to June 2015,including 13 males and 2 females;the age of patients were 36 to 84 years old,averaged 59.6 years old.Cause of injury:high falling injury in 4 cases,traffic accident in 4 cases,bruise in 2 cases,falling in 5 cases.All patients were examined by posteroanterior and Lateral cervical X-ray,cervical CT and MRI scans,and all patients were treated by posterior cervical unilateral open-door laminoplasty with lateral mass screw fixation.Compare the cervical JOA score,VAS score,C2-7 Cobb angle and vertebral slip distance of patients at final follow-up after operation with pre-operation.And examine the cervical range of motion at final follow-up after operation.ResultsAll patients were followed up from 12 to 20 months,averaged 15.6 months.The cervical JOA score,VAS score,C2-7 Cobb angle and vertebral slip distance of patients at final follow-up after operation were significantly better than pre-operation,and the differences were statistical significance(p<0.05).The patients had preferably cervical range of motion after operation,and there were not found the patients with complications such as internal fixation loosening or breakage.ConclusionsThe patients with CSM and cervical instability were treated by posterior cervical unilateral open-door laminoplasty with lateral mass screw fixation could decompress cervical cord and get better cervical stability,it is a safe and effective operation.
