Objective:To investigate the diagnostic value of capsule endoscopy in patientswith gastrointestinal diseases of unknown causes, and guide the reasonableapplication of capsule endoscopy.Methods:The clinical data of251patients underwent OMOM capsuleendoscopy with telephone follow-ups were retrospectively analyzed.Results:1. Reasons for applying CE test: There were significant differences inthe patients with obscure gastrointestinal bleeding between the elderlygroup and the non-elderly group (51.9%vs.27.3%). Meanwhile, there weresignificant differences in the patients with abdominal pain or diarrheabetween the elderly group and the non-elderly group (48.1%vs.67.3%).2. Transit condition of CE: The complete small-bowel examinationgroup had a significantly shorter gastric transit time than the non-completesmall-bowel examination group (42.3min vs.60.6min). The rate ofcomplete small-bowel examination was93.9%. CE showed that the risk ofnon-complete small-bowel examination was bile reflux (OR=4.70). Metoclopramide (10mg, intramuscular injection) did not exerted effect onthe small bowel transit time (298.0min vs.324.6min), but it can reducethe gastric transit time of the capsule.3. Results of CE: The detection rate of small bowel diseases wassignificantly higher in the high definition capsule endoscopy group thanthat in the06capsule endoscopy group (94.3%vs.86.4%). There were nosignificant differences in the detection rate of small bowel disease amongthe elderly group, the children group and the middle-aged group (90.1%vs.100%vs.91.0%). The positive detection rate of small bowel diseases wasup to95.0%in the patients with obscure gastrointestinal bleeding (95/100),most commonly in vascular malformation (28.0%). The positive detectionrate was44.4%(8/18) for high risk population of small intestineancylostomiasis;86.5%(160/185)for abdominal pain or diarrhea groupwith mucosal inflammation was frequent seen (29.7%). The positivepredictive value was86.8%(33/38).4. All patients were well tolerated, only1case happen capsuleretention.Conclusions:The manipulation of capsule endoscopy is effective, safe and simpleand it is of important value in the diagnosis of small intestinal diseases.Metoclopramide (intramuscular injection) improved the complete rate.Parasite elimination can be done before CE exam for high risk population of small intestine ancylostomiasis.
