Objectives:1. By investigating the expression differences of matrix metallo-pmteinase-9(MMP-9) among oral submucous fibrosis (OSF), including initial, medium-term and advanced stage, and normal buccal mucous(NBM), exploring the possible role of MMP-9in the occurrence and development of OSF.2. By investigating the expression differences of MMP-9among OSF coexisted with OLP, non-erosive oral lichen planus(OLP) and OSF, discussing the expression of MMP-9in the various lesions, and the correlation between MMP-9and the clinical characteristics of trismus and so on in OSF patients coexisted with OLP.Methods:The expression of MMP-9by immunohistochemical SP method in33cases of OSF (comprises of11initial,11medium-term and11advanced stage cases),15cases of OSF coexisted with OLP,15cases of non-erosive OLP and10cases of NBM. The different positive cell rates and expression intensity of MMP-9in the six groups were compared by statistics analysis.Results:1. NBM showed negative expression of MMP-9. There were obvious expressions of MMP-9in OSF, OSF coexisted with OLP, non-erosive OLP. Positive brown staining was mainly located in plasm and membrane of stratum basale cells, stratum spinosum cells and lamina propria infiltrating inflammatory cells.2. The differences in positive expression rates of MMP-9between OSF (all stages were45.5%), OSF coexisted with OLP(86.7%), non-erosive OLP (93.3%) and NBM (0) were statistically significant (P<0.05). The positive expression rates of MMP-9between all stages of OSF and OSF coexisted with OLP, non-erosive OLP also had a significant difference. But there were no significant differences in positive expression rates among the three stages in OSF (P>0.05). No statistically significant difference in positive expression rates was seen between OSF coexisted with OLP and non-erosive OLP (P>0.05).3. The differences in expression intensity mean of MMP-9between OSF (initial OSF was1.09±1.29, medium-term was2.18±1.96, advanced stage was2.27±2.12), OSF coexisted with OLP(3.47±2.27), non-erosive OLP(4.47±1.69) and NBM(0.3±0.23) were statistically significant (P<0.05). The expression intensity mean of MMP-9between all stages of OSF and OSF coexisted with OLP, non-erosive OLP also had a significant difference (P<0.05). There were statistically significant differences in expression intensity meaning between initial OSF and medium-term, advanced OSF (P<0.05). However, the comparison between medium-term OSF and advanced OSF did not provide any statistically significant difference (P>0.05). The expression intensity mean of MMP-9in non-erosive OLP was higher than that of OSF coexisted with OLP, the difference was statistically significant (P<0.05).Conclusions:1. The expression of MMP-9in oral mucosa of OSF patients was higher than NBM, suggesting that MMP-9had a relationship with pathogenesis of OSF.2. The expression of MMP-9in OSF coexisted with OLP was higher than that in OSF, lower than that in non-erosive OLP, which might be one of the reasons why the OSF patients coexisted with OLP did not appear fiber rope, trismus and other clinical features.
