Objective:To explore the prevalence of high-risk papillomavirus (HR-HPV) and age distribution in gynecological outpatients, to investigate the distribution of HPV load with different levels of cervical lesions and the distribution of different human papillomavirus (HPV) genotypes.Methods:Collect a total of8734cases who had high-risk HPV testing in Qilu Hospital of Shandong University gynecological clinic between March2008to March2012,2604cases were positive. Part of HPV-positive patients underwent HPV typing test,a series of225patients diagnosed by cytological examination, HR-HPV detection by HC2,colposcopy and biopsy under the colposcopy.Results:1Prevalence of HPV DNA in gynecological outpatients was29.81%,with the highest prevalence (48.77%)among women aged16to25years,its infection rates declining with age, the positive rate of36-50years old women is30.79%which is another peak of infection, the infection rate of the age after50years old is25.34%.2. There was no significant difference in the incidence age between different levels of CIN, the incidence age of patients with cervical cancer was significantly higher than that of precancerous lesions.3.In screening CIN2or above,sensitivity,specificity, positive predictive value (PPV)and negative predictive value (NPV) of HC2were92.22%,53.49%,67.48%86.79%, its sensitivity and NPV was higher than cytology and the misdiagnosis rate(11.9%) was lower than cytology(20.55%) with low specificity.4. In screening CIN2or above, sensitivity,specificity, PPV, and NPV of HC2 combined cytology were98.89%、70.93%、78.07%、98.39%,its sensitivity and specificity was higher than those tested alone,while reducing the misdiagnosis rate of the high-level lesions (2.56%).5. The infection rate of HR-HPV is ascending with serious degree of cervical lesion.The load of HR-HPV in cervical cancer is higher than precancerous lesions and there is no significantly difference betweem CIN1-CIN3.6The most prevalent types of human papillomavirus in gynecological outpatients in our hospital are HPV16,58,18,32, single and multiple infection rate was67.62%and32.38%。Conclusions:1. Prevalence of HPV DNA is age-related, with the highest prevalence (48.77%)among women aged16to25years and another peak of infection is36to50years (30.79%), its infection rates declining with age.2. There was no significant difference in the incidence age between different levels of CIN, the incidence age of patients with cervical cancer was significantly higher than that of precancerous lesions.3. HR-HPV detection is an important method in screening cervical lesion,HC2combined with cytology can improve sensitivity and NPV with low misdiagnosis rate.4The most prevalent types of human papillomavirus in gynecological outpatients in our hospital are HPV16,58,18,32,...
