Objective:Development of cervical cancer is associated with many factors,among them, genetic background play a significant role.Several reports indicate that estrogen receptorαgene polymorphism might have a relation with many kinds of tumor susceptibility.However,there have no studies about the report between estrogen receptorαgene polymorphism and cervical cancer.In this study,we would make a penetrated approach of the relation between estrogen, ERαPvuâ…¡å'ŒXbaâ… gene polymorphism and cervical cancer.Method:A hospital-based case-control study was conducted.102 cases with cervical cancer and 125 controls with gynecology benign tumor by pathological diagnosed from Shanxi Tumor Hospital were involved in this study from September,2004 to September,2005.All participants were Han nationality and had inhabited in Shanxi province for more than twenty years.The pregnant,previous and present ovaries patients,other cancer patients,steroid hormone users or psychotic patients were excluded.All cases and controls were investigated directly by medical staff with the same questionnaires.The questionnaires included basic information,education and other socioeconomic factors;general lifestyle habits such as smoking and alcohol consumption; sexual behavior and heath habits;menstrual,marriage and pregnancy history;medical history and tumor family history.Peripheral blood sample were collected from all participants in NO.8~10 day of menstrual cycle.Estradiol was detected by ELISA assays,ERαPvuâ…¡and Xbaâ… gene polymorphism were determined by PCR-RFLP method.All data were analyzed by SPSS13.0,usingχ2-test,t-test,univariate and multiple non-conditional logistic regression analysis test the association between the level of E2,ERαPvuâ…¡and Xbaâ… genetype,correlation factors and cervical cancer.Result:(1)The result of sample factor analysis showed that culture,occupation,smoking, state of menses,age at first sexual intercourse,age at first pregnancy,age at first procreate,times of pregnancy and times of parity were all associated with cervical cancer;(2)Estradiol of case(47.57±24.27pg/ml)was more higher than control(47.57±24.27pg/ml),with statistic significance between two arrays;(3)The analysis of interaction between the state of menses and estradiol level to cervical cancer indicate there was interaction between two factors;(4)The distribution of ERαPvuâ…¡and Xbaâ… gene polymorphism consistent with Hardy-Weinberg balance;(5)Comparing the frequencies of homozygosis mutation type Xbaâ… XX genotype, heterozygosis mutation type Xx genotype to wild type xx genotype of case and control,the difference between Xbaâ… XX genotype and Xbaâ… xx genotype had no statistic significance(χ2= 0.573,P=0.449),but there was statistic significance between Xbaâ… Xx genotype and Xbaâ… xx genotype(χ2=4.372,P=0.037);merging the frequencies of Xbaâ… XX and Xx genotype, comparing with Xbaâ… xx genotype,and the difference had statistics significance(χ2=7.870,P= 0.005),OR=0.655(95%CI,0.484~0.887),occurring the mutation of Xbaâ… gene might decrease the risk of cervical cancer;(6)Comparing the frequencies of homozygosis mutation type Pvuâ…¡PP genotype,heterozygosis mutation type Pp genotype to wild type pp genotype,the difference between case and control had no statistic significance,merging Pvuâ…¡PP,Pp genotype and compared with pp genotype,there was still no statistic significance(χ2=1.059,P=0.304);(7) There had synergism effect between ERαXbaâ… gene mutation and E2 level no heightening,the mutating of ERαXbaâ… gene would decrease the risk of the cutting effect for E2 depressing;(8) Occupation,smoking,E2 and times of parity were involved in multi-factors logistic regression model.Conclusion:(1)Smoking,young age at first sexual intercourse and first pregnancy,more times of pregnancy and parity were the risk factors of cervical cancer,but high level of education background,engaged in brainwork,post menopause might the protected factors of cervical cancer;(2)Higher level of E2 might be the potential risk factor of cervical cancer;(3)ERαXbaâ… gene mutation play important roles in the development of cervical cancer;(4)There had synergism effect between ERαXbaâ… gene mutation and E2 level no heightening.
