Objectives: To establish the model of intrapelvic perfusion with SIRS in pigs, andobserve the curative effect of Xuebijing injection for intrapelvic perfusion of SIRS in pig,and investigate the mechanism, so that to provide the experimental and theoretical basis fortreatment of PCNL with SIRS after operation.Methods:Fifteen pigs were randomly divided into sham group, model group and XBJgroup. Each group was five, and further divided into five observation points according tothe time of preoperative, postoperative0h,6h,12h and24h. Model group: The SIRS ani-mal models were established by joining into the pelvis flush perfusion fluid with autolog-ous cecum contents; Sham group were opened abdominal cavity and tumbled intestinalcanal only, and without pelvis perfusion; XBJ group: Xuebijing injection50mg plus nor-mal sodium100ml was injected through ear vein tiwce a day after modeling. Sham groupand Model group input normal sodium100ml at the same time. Rectal temperature (RT),respiratory rate (RR), heart rate (HR); Peripheral blood cell counts (WBC, RBC, PLT),renal function test (Cr, BUN, β2-MG), and the serum levels of IL-6, IL-10and TNF-α wererecord and analyzed; the pathological changes of kidney tissue were observed by opticalmicroscope and electron microscopic.Results:1. The change of general index: There was no marked change before and af-ter operation in sham group, and RT, RR, HR, WBC and RBC significantly increased butPLT decreased gradually in model group. The above indexes were increased in XBJ groupat6h,12h and24h, but the degree of this increase was much lower than the model group(P<0.05).2. The change of biochemical indicator: Cr, Bun and β2-MG of sham group postoper-ative were increased slightly but there were no significant difference (P>0.05), Cr、Bun、β2-MG increased gradually in model group, compared with preoperation, the differencewas statistically significant(P<0.05). The above indexes were increased in XBJ group at6h, 12h and24h, but the degree of this increase was much lower than the model group(P<0.05).3. The change of IL-6, IL-10and TNF-α: IL-6, IL-10and TNF-α of sham grouppostoperative were increased slightly but there were no significant difference (P>0.05). InModel group, TNF-α reached its peak value at6h (226.02±27.45) pg/ml, IL-6reached itspeak value at12h (320.40±85.20) pg/ml and IL-10reached its peak value at12h(547.01±76.32) pg/ml. The above indexes were increased in XBJ group at6h,12h and24h,and the degree of this increase was much higher than the model group (P<0.05).4. Morphological changes: By light microscopy, the glomerulus were normal, tubulirenales lumen were round and integrity in Sham group. After modeling, renal injuries withcellular swelling, cast formation and inflammatory cells infiltration in renal interstitiumwere found. By transmission electron microscopic, there were no significantly differenceson the kidney structure, but in Model group, after modeling, glomerular podocytes ap-peared cellular fusion and cell loss, mesangial cell and endothelial cell appeared hyperpla-sy and edema. Although XBJ group appeared above pathology change also, but the degreehas reduces.Conclusions:1. The SIRS animal models established by joining into the pelvis flushperfusion fluid with autologous cecum contents were successful.2. Xuebijing injection can effectively reduce the serum levels of TNF-α, IL-6andIL-10, block the inflammatory cascade, maintain the balance of SIRS and CARS. It hasmarked effect on the SIRS pig.
