Objective:To study the impact of eliminating phlegm and dampness law to regulate menstruation on the patient who suffers polycystic ovarian syndrome (PCOS) with hyperandrogenism.Methods:This research contains the theory research and clinical cases. In the theory research part we illustrate the pathogenesis and treatment progress at present of PCOS systematically through reading and sorting the ancient books and records. And we also expound for the pathogenesis, pathophysiology, diagnosis, and treatment of PCOS. In the clinical research part we choose30patients who suffer PCOS with hyperandrogenism(type of stagnation of phlegm-damp) in their own control study, administrate drugs for eliminating phlegm and dampness to regulate the menstrual function to them, and add or subtract the dosage by their symptom. After three courses of treatment (3months for a course of treatment), we observe the LH, FSH, T, E2, and SHBG in serum before and after the treatment. The Ferriman-Gallwey score standard is used to assess hairy signs, the Rosenfeild score standard estimate acne score. We also measure and record body mass index and waist-hip ratio. All above are used to explore the mechanism of eliminating phlegm and dampness law to regulate menstruation on the patient who suffers PCOS with hyperandrogenism.Results:The age of patients who suffer PCOS with hyperandrogenism focus on child-bearing period, the age range is25.70+/-3.67years old. In30patients with eliminating phlegm and dampness law to regulate menstruation,3cases are cured,11cases effective,14cases less effective and2cases effectiveness. The total effective rate is93.33%. In10cases with infertility as the chief complaint,4cases get pregnancy. The drug side effects rate in30patients is13.33%, and the main side effects are breast pains (2patients) and abdominal distension (2patients). The TCM symptom score crition post-treatment in PCOS patients reduce significantly (P<0.05) compared with those prior treatment. The BMI and WHR post-treatment in PCOS patients reduce more significantly (P<0.01) compared with those prior treatment. The LH and LH/FSH decline remarkably too (P<0.01), while there is no significant difference (P>0.05) though FSH rise. The E2and PRL dosage have no significant difference (P>0.05). The blood T (P<0.05)and SHBG (P<0.01) decreased with significant difference. The patient’s hairy sings and acne symptoms release with significant difference (P<0.01). Compared with that prior treatment, the patient’s follicular count of unilateral or bilateral ovarian reduce remarkably (P<0.01).Conclusion:The eliminating phlegm and dampness law to regulate the menstrual function has an obvious efficacy on PCOS with hyperandrogenism. It can reduce blood LH and T value, rise FSH value, and reduce LH/FSH ratios. It also depresses free androgen content in blood circulation through reducing the SHBG. It provides a effective treatment strategy for PCOS because of it’s light side effects, satisfactory tolerance and clinical efficacy. |
