ObjectiveTo evaluate the clinical efficacy of treating acute gouty arthritis with eye acupuncture and opposing needling method combined with distal needling method in order to explore a treatment with rapid analgesic efficacy.MethodsUse simple randomized controlled trial method and recruit60patients of acute gouty arthritis who met the inclusion criteria from the inpatient and outpatient department of Acupuncture Department and Rheumatology Department. of Guangdong Provincial Hospital of Traditional Chinese Medicine.60cases randomly divided into two groups,29cases for treatment group who accepted eye acupuncture treatment,31cases for control group who accepted opposing needling method combined with distal needling treatment, the treatment group to the eye acupuncture treatment. Methods:(1) treatment group: to use the eye acupuncture and choose either the upper energizer zone or lower energizer zone according to the location of the joint involved.(2) conLrol group: to use the combination method of opposing needle and distal needle, following the crossing and corresponding principle of "left-right and up-down" to choose the points. Treatment:6times account for one course,all the treatments are required to he completed within one week.①To evaluate the VAS score respectively and the difference between two groups at3time points: before the treatment, five minutes after the first treatment, at the end of the treatment.②To evaluate the difference of analgesia onset time and analgesic efficacy maintain time between the two groups.③To evaluate the difference of degree of joint swelling and uric acid between Lhe two groups before and at the end of treatment, and to keep records of any adverse reactions and side effects during the study, using statistical software SPSS18.0and collected da t a t o do statistic al analysis.Results1.To compare pro-treatment baseline between the two groups in terms of gender, age, disease duration, education level, TCM type, prc-treatment VAS score, joint swelling score, uric acid, etc. The differences were not statistically significant (P>0.05), which shows that both groups are homogeneous, balanced and comparable.2. By doing paired and independent sample rank sum test analysis, joint swelling scores of both groups decreased during treatment, but, the difference between the groups was not statistically significant (P>0.05), which shows both groups can effectively relieve joint swelling, but the effect is almost the same.3.By doing paired and independent sample t-test analysis, uric acid change of patients treated with eye acupuncture shows a downward trend, but the difference before and after treatment was not statistically significant (P>0.05); uric acid change of patients from control group shows an upward trend, which shows both interventions are invalid to reduce uric acid.4. Analgesic efficacy:by doing the independent sample rank sum test, difference of analgesic onset time of initial treatment was significant statistically significant (P<0.01), the average rank of treatment group is less than that of control group, indicating that the analgesic onset time of treatment group is faster; By doing independent samples t-test analysis, the difference of analgesic maintain time of the two groups was significant difference statistically significant (p<0.01), indicating that the treatment group has a better efficacy in maintaining analgesic effect.5. By doing repeated measurements multivariate AN0VA analysis, VAS scores changes of both groups show a downward trend during treatment and the trend difference was statistically significant (P<0.01), indicating that the treatment group and control group can both relieve joint pain effectively.6. By doing repeated measurements through multivariate analysis of variance between groups, difference was statistically significant (P<0.01). For comparison of the mean scores of the two adjacent points of both groups, difference between treatment group and control group during treatment was statistically significant (P<0.01). These results suggest that the analgesic effect of the treatment group is better than control group.7. There are no side effects and serious adverse reactions during the course of the study.Conclusion1. Both of eye acupuncture and opposing needling combined with distal needling method are effective to alleviate the acute gouty arthritis, joint pain and joint swelling.2.
