Objecti veAim to observe the clinical efficacy that conventional treatment combine with traditional Chinese medicine and BAL on the patient with AECOPD(phlegm obstruct the lung) which use the airway mucus hypersecretion as a steating point.MethodsThis study used a randomized controlled trial, selection patients with a strict criteria from ICU and respiratory department of Guang Dong Province Chinese Medicine Hospital during December2010to March2012, observation the basic treatment group (group A), basic treatment and traditional Chinese medicine group(group B), basic treatment, traditional Chinese medicine and BAL treatment(group C) in patients with AECOPD before and after treatment, including their TCM syndrome score, the expectoration capacity, blood gas analysis, bloodconventional, procalcitonin, C-reactive protein changes, and observe the safety index of the two will be routine, ECG, liver and kidney function.Result1. The baseline conditions:Three groups of patients’gender, age, disease duration, disease severity before treatment, TCM syndrome score, sputum traits were not statistically significant (P>0.05), among the three groups were comparable.2. TCM symptoms and signs:After treatment, three groups of patients with cough, expectoration, signs and symptoms of wheezing, pulmonary rales, expectoration capacity compared with pre-treatment, it had a significantly improved, the results were statist icall y significant (P<0.05). Between the three groups, group C clinical control rate was significantly improved compared with group A, the difference was statistically significant:(P<0.05), group C expectorant effect significant efficiency and overall efficiency compared with group A and group B, its results with significant difference (P<0.05).3. Laboratory parameters:Group C in the treatment of white blood cell count, neutrophil percentage, C-reactive protein, procalcitonin than those pre-treatment significantly improved, the results were statistically significant (P<0.05). Group B in the treatment of white blood cell count, neutrophil percentage of cells and C-reaclive protein treatment improved with a significant difference (P<0.05). Group A after treatment, percentage of neutrophils and C-reactive protein had improvement, the difference had statistically significant (P<0.05). About blood gas analysis, three groups in the treatment of PH, PaC02, SaO2%had improved signi ficantly, the results were statistically significant (P<0.05)ConclusionBased on pre-treatment of Western medicine, tradi tional Chinese medicine and bronchoalveolar lavage had a better improve on AECOPD(phlegm-heat Yong lung) patients. Its treatment of one of the mechani sms may through the Chi nose medicine phlegm and then improve the state of airway inflammation and mucus hypersecretion, Joint use of the bronchoalveolar lavage of normal sal ine can appeal directly to the viscous sputum and clean up sputum, it can remove respiratory secretions and congestion, enhance local irritation, cnhanced cough reflex, and conducive to the discharge of the distal small airways and alveolar inflammatory secretions, thereby improving ventilation ventilation. Its efficacy is superior to Western medicine treatment group.
