Chronic obstructive pulmonary disease (COPD) is a preventable and treatable disease state characterized by an airflow limitation that is not fully reversible. The airflow limitation is usually progressive and associated with an abnormal inflammatory response of the lung to noxious particles or gases. It is a major public health problem due to high morbidity and mortality. It has become a heavy social economical burden.COPD, which was not recognized in traditional Chinese medical literature, is comparable to Pulmonary Distention (PD) in Chinese medicine. It is commonly suggested that the causations of PD are pulmonary asthenia resulted from long-term illness. PD will be deteriorated progressively in case of frequent external contraction pathogen. Lung is the first organ injured in the process of PD, and then, the spleen, kidney, heart, and liver. The pathological mechanisms underlying PD are qi asthenia, vacuity of both qi and yin, dual vacuity of yin and yang. The pathological manifestations of PD include phlegm, drink, and stasis blood. Qi asthenia, stasis blood, and phlegm obstructing are mainly pathological basis of PD.The summary mainly summarized the cause, the pathologic mechanism, the syndrome and the therapy of COPD by reference to literature about Traditional Chinese Medicine.Methods:The research brought into 299 patients who comply with the diagnosis standard. Methods of retrospective study were adopted in the study. The course of disease, pulmonary function, arterial blood gas analysis, and the relativity between these indexes above-mentioned and the different Traditional Chinese medicine (TCM) deficiency Syndrome type were analyzed. The objective regularity of TCM pathogenesis in AECOPD and the relationship between it and those modern medicine indexes were approached. It was to provide objective basis for the TCM Syndrome type of AECOPD relatively.Generally speaking, there are quite a lot of deficiency syndromes in all the case. Deficiency of Fei-Qi is of predominance. The results indicated that the course of disease increased gradually (P<0.05) from Fei-Qi Deficiency, Fei-Pi-Qi Deficiency, Fei-Shen Deficiency to multi-organ Deficiency; and the rate of obstructive emphysema, chronic cor pulmonale increased (P<0.05); the level of pulmonary ventilation function declined, the level of RV/TLC ascended gradually (P<0.05); the level of PaO2 declined, the level of PaCO2 ascended (P<0.05).Conclusions:The deficiency syndromes of AECOPD are characterized as complex patterns, which have the basic characteristic of the deficiency of Fei-Qi. The study disclosed that the course from Fei-Qi Deficiency, Fei-Pi-Qi Deficiency, Fei-Shen Deficiency to multi-organ Deficiency was a progress of disease from light to serious by the observation of indexes in AECOPD. And the outcome provided objective basis for TCM deficiency syndromes type of AECOPD relatively.
