Background and ObjectiveMissed abortion is one of the common pathologic pregnancy in obstetric.The etiology is complicated, and pathogenesis is not yet clear. Reproductive immunology thinks missed abortion is immune imbalance in maternal-fetal interface, maternal tissues engender immune rejection to fetal tissues.TLR4is an important kind of pattern recognition receptors in the innate immune system, The study found that trophoblast cells can express TLR4. TLR4through a series of transmembrane signal transduction to activate NF-κB, cause synthesis and release of a large number of inflammatory factor, and play a role in innate immunity and acquired immunity.Recent stuty found that HBD2may have activity of direct killing bacteria and virus, play an important role in the natural immunity of the villi, at the same time play a key role in the maintenance of pregnancy.The pathogenesis of missed abortion has been a hotspot in the field of obstetrics at home and abroad, however,,HBD2and TLR4has been reported less in pregnancy and pathological pregancy, especially HBD2.In order to explore the expression of TLR4, NF-κB, HBD2in villi tissues between missed abortion patients and normal women with early pregnancy, RT-PCR and ELISA were used to detect mRNA expressions and protein expressions, in order to explore the role of TLR4, NF-kB and HBD2in the pathogenesis of missed abortion.Materials and Methods1.60women from the First Affiliated Hospitaof Zhengzhou University were randomly seleeted accordance with inlusion criteria and divided into two groups:(1) missed abortion women as abortion group (n=30),(2) normal early pregnancy women as healthy pregnancy group (n=30). villi tissues were btained during artifieial abortion.2. Reverse transcription-polymerase chain reaction (RT-PCR) was used to detect mRNA expressions of TLR4, NF-κB and HBD2, enzyme-linked immunosorbent adsorption assay (ELISA) was used to detect protein expressions of TLR4and HBD2, and do correlation analysis.3. All datas were analysised by SPSS17.0statistical software. According to the normal distribution and homogeneity of variance test results, T-test was used to analy the ststistical methods of measurement datas. Pearson correlation was used to do correlation analysis. Inspection level was α=0.05, P<0.05meaned that the difference was statistically significant.Results1. The mRNA expression of TLR4in abortion group (1.01±0.14) was higher than healthy pregnancy group (0.70±0.12), the difference was statistically significant (P<0.01)2. The mRNA expression of NF-κB in abortion group (0.83±0.11) was higher than healthy pregnancy group (0.77±0.11), the difference was statistically significant (P=0.020)3.The mRNA expression of HBD2in abortion group (0.44±0.05) was higher than healthy pregnancy group (0.40±0.06), the difference was statistically significant (P=0.003)4. The protein expression of TLR4in abortion group (5.47±0.57) ng/g was higher than healthy pregnancy group (4.93±0.74)ng/g, the difference was statistically significant(P=0.002)5. The protein expression of HBD2in abortion group (3.28±0.74) ng/g was higher than healthy pregnancy group (2.73±0.65) ng/g, the difference was statistically significant (P=0.003)6. In abortion group, the mRNA expressions of TLR4, NF-κB were positive correlation (r=0.638, P<0.01), the mRNA expressions of TLR4, HBD2were positive correlation (r=0.723, P<0.01), the mRNA expressions of NF-κB, HBD2were positive correlation (r=0.532, P=0.002), The protein expressions of TLR4and HBD2were positive correlation (r=0.813, P<0.01).In healthy pregnancy group, the mRNA and protein expressions of TLR4, NF-κB, HBD2have no correlation (P>0.05)Conclusion1. The expressions of TLR4, NF-κB and HBD2in missed abortion women was higher than in normal early pregnancy women.2. Increased expressions of TLR4, NF-κB and HBD2may play an important role in the pathogenesis of missed abortion.3. Linear positive correlation of TLR4, NF-κB and HBD2indicated that TLR4/NF-KB/HBD2signal transduction pathway may be involved in the development of missed abortion.This may be an important pathogenic mechanism.
