Objective:By observing changes in behavior, spinal cord tumor necrosis factor alpha and plasma prostaglandin E-2after rat plantar incision, we investigate the possible function mechanism of dezocin and parecoxib sodium which are used as the analgesic thus providing the basis for combination of dezocine and parecoxib sodium which is adopted for the purpose of postoperative pain relief.Methods:32Wistar rats were randomly divided into four groups(n=8):the control group(Group DoPo), the dezocine group(Group D1P0), the parecoxib sodium group(Group D0P1), the combined group(Group D1P1).Group DoPo (n=8):incision pain model, tail vein injection of saline; Group D1P0(n=8):incision pain model, immediate tail vein injection of dezocine5mg after surgery.Group D0P1(n=8):incision pain model, immediate tail vein injection of parecoxib sodium10mg/kg after surgery.Group D1P1(n=8):incision pain model, immediate tail vein injection of parecoxib sodium10mg/kg and dezocine5mg after surgery.Stepping behavior was evaluated0-60min after hind paw incision.Two hours later, rats were extracted blood from vena cava.The spinal cord enlargement was removed at2h after awake.The content of TNF-α and PGE-2was measured by using ELISA.Result:1.Compared with Group DoPo, rats in Group D1P0, D0P1and D1P1had lower cumulative pain scores (P<0.05); compared with Group D1P0, rats in Group D0P1had higher cumulative pain scores (P<0.05), and rats in Group D1P1had lower scores (P<0.05); compared with Group D0P1, rats in Group D1P1had lower scores (P<0.05)2.Compared with Group D0P0, the TNF-a expression of Group D1P0and D1P1became lower(P<0.05); in Group D0P1, the decline of TNF-a expression was not obvious (P>0.05); compared with Group D1P0, the TNF-α expression of Group D0P1increased (P<0.05) and that of Group D1P1declined (P>0.05); compared with Group D0P1, the TNF-a expression of Group D1P1declined (P<0.05)3.Compared with Group D0P0, the levels of plasma PGE-2of Group D1P0, Group D0P1and D1P1were significantly lower (P<0.05); compared with Group D1P0, plasma PGE-2 levels of Group D0P1and D1P1were lower(P<0.05); compared with Group D0P1,the decline of plasma PGE-2levels of Group D1P1was not obvious (P>0.05)Conclusion:1.Intravenous injection of dezocine can inhibit the incisional pain in rats, reduce the spinal cord of TNF-alpha expression and lower plasma levels of PGE-2in rats.2.Parecoxib sodium intravenous injection can inhibit incisional pain in rats and reduce the levels of plasma PGE-2significantly, but the its effect for inhibiting spinal cord TNF-α expression is not obvious.3.The combination of dezocine and Parecoxib sodium can obviously inhibit the incisional pain in rats, reduce the spinal cord of TNF-alpha expression and lower the plasma levels of PGE-2in rats.4.Dezocine can play the analgesic effect by inhibiting the expression of pro inflammatory cytokines and pain signal transduction.Parecoxib sodium can inhibit hyperalgesia by inhibiting central and peripheral levels of PGE-2.The combination of dezocine and parecoxib sodium can achieve a better analgesic effect, so it is feasibility for clinical application.
