Objective To understand the current health emergency situation among health administrations at all levels (province-city-county), centers for disease control and prevention (CDC), health monitoring institutions and medical institutions (above Class2) and to study the influence factors of health emergency capability. To improve the system construction of regional health emergency capability.Methods Using the basic unified national health emergency questionnaire which created by Ministry of Public Health of China, the census method was taken to investigate the health emergency situation in all provincial health administrations at all levels (province-city-county), centers for disease control and prevention, health monitoring institutions and medical institutions (above Class2).242questionnaires were distributed and232valid questionnaires were withdrawn. The response rate was95.9%, of which,53from health administrative departments,52from the centers for disease control and prevention,53from health authorities,74from medical institutions. The content of the investigation included institutional settings, the team construction, training exercises, equipment and reserves, working expenses and reserves, etc. The evaluation index was constructed to analysis the current situation of health emergency work by using SAS statistical software. The principal component analysis method was performed to evaluate health emergency response capacity in the health administration of the province’s counties including health administration, disease prevention and control, health supervision and medical institutions (above Class2) and to perform the exploratory univariate analysis of the relevant factors about the emergency response capacity.Results At present, the health emergency system has been preliminarily contructed in all levels of health department in Qinghai province. Response Planning rate was88.8%, the average response planning number was5.84.1%of the health institutions has set up the health emergency team, an average of2.1kinds and2.5. In2009the average drill number was1.3times, the average training number was2.5times, only12.5%of the health institutions had completed the full training.69.4%of health institutions has emergency supplies reserves,44%of health institutions has a small amount of daily health emergency funds.29health institutions provide the risk subsidies to emergency players, only7medical institutions had bought insurance for emergency players. The survey found the health emergency capacity has regional differences in Qinghai province, Xining city and Haidong Prefecture with the economic developed area were best, Huanhu region was better,compared to Qingnan region.40.1%of the institutions has established health emergency coordinate departments, but have only provincial health department of fixed home1, no emergency command system. Health emergency command and coordinated management system should be perfect; Emergency team training system is not strong; lacked of long-term emergency materials reserve system and health emergency special protected funds; and Overall health emergency response capacity need to be improved.Conclusions According to theoretical framework of "a case of three system", it has established the system of plans for the emergency construction of public health emergencies and the corresponding legal system, system, mechanism construction; has established public health emergencies emergency command center entity as soon as possible and has implemented the basic health emergency response agencies establishment. The health sector should develop the planning of emergency response training, systems training; drill number and quality, improve basic health emergency professional competence; to strengthen public health emergencies hindsight assessment; emergency special funds to support and emergency supplies reserves and long-term mechanism is the key to enhance the health emergency response capacity.
