Objective:To investigate the effects of intensified diet and exercise intervention on type2diabetes mellitus glucose and lipid and intima-media thickness and cost-effectiveness analysis, and to provide a reference for lifestyle intervention in the treatment of diabetes and for the health authorities to develop the optimal health-care resource allocation.Methods:58cases of type2diabetes were divided into two random groups, that is, the intervention group and control group. There are32cases in the intervention group, and they were received diet and exercise prescription by "Proper Diet Is Easy To lower Blood Glucose" and "Diabetes Quantized Diabetes Exercise Prescriptio" on the conventional drug therapy basis. There are26cases in the control group, and they were received only general verbal guidance on diet and exercise in addition to the prescription application of drug therapy. Comparison on the blood glucose, blood lipids and blood pressure changes of the two groups before and after the intervention at different time periods of3,6,9,12months. Indicators change of each group before and after the intervention of four different time periods. To compare intima-media thickness (IMT) before and after intervention of12months periods. Vsing the cost-effectiveness analysis for evaluation of health economics.Results:(l)After intervention of3months periods, glycosylated Haemoglobin (HbAlC) of the intervention group was significantly lower than control group (P<0.05); After intervention of6months periods, fasting blood glucose (FPG),2-hour postprandial blood glucose (2hPBG), HbAlC, total cholesterol (TC) were significantly lower than control group (P<0.05). After intervention of9months,12months periods, FPG,2hPBG, HbAlC, TC, low density lipoprotein cholesterol (LDL-C), systolic blood pressure (SBP), diastolic blood pressure (DBP) were lower than control group (P<0.05). For the intervention group,after intervention of3months,6months,9months, and12months periods, FPG,2hPBG, HbAlC, TC, triglyceride (TG), LDL-C, SBP, DBP of the intervention group were significantly lower than before (P<0.05). After intervention of6months,9months, and12months periods, the high density lipoprotein cholesterol (HDL-C) was much higher than before (P<0.01). For the control group, after intervention of3months and6months periods,2hPBG, HbAlC and TG were significantly lower than before (P<0.01), and FPG, TC, LDL-C, HDL-C were no significant change (P>0.05); after intervention of9months periods, HbAlC was lower than before (P<0.05)and HDL-C was higher than before (P<0.01) and FPG,2hPBQ TC,TG, LDL-C, SBP, DBP were no significant change (P>0.05); after intervention of12months periods,2hPBG and HbAlC were lower than before (P<0.01) and HDL-C was higher than before (P<0.05) and FPG, TC, TG, LDL-C, SBP, DBP were no significant change compared with before (P>0.05).(2)After intervention of12months periods,the intervention group intima-media thickness compared with the previous was decreased(P<0.05), and there was no significant change in control group (P>0.05). After intervention of12months periods,there was no significant change between the two groups’s intima-media thickness (P>0.05).(3) The medical cost of the intervention group used for diabetes treatment and diabetes complications was significantly lower than control group (P<0.05), and the treatment effect was better than the control group. Cost-effectiveness ratio of the intervention group was lower than the control group, and compared with intervention group, the control group needed medical additional costs for every effect of each glucose and lipids additional units.Conclusions:(1) Intensified diet and exercise therapy can be the fastest, and more stable way to control of blood glucose, blood lipids, blood pressure, and the effect is significant.(2) Intensified diet and exercise therapy can delay carotid artery intimal thickening.(3) Intensified diet and exercise therapy can reduce medical costs.
