Objective:Study the levels of endothelin (ET) and high-sensitivity c-reactive protein (hs-CRP) in type 2 diabetic patients with hypertension and related factors, and to explore its role in the pathogenesis.Method:We selected 80 cases of type 2 diabetes patients with hypertension (DM combined HP group) and96 cases of pure type 2 diabetes (DM) from our hospital in October 2010 May 2011, and 60 normal controls. We tested the serum levels of ET, hs-CRP, FBG, TG, TC, LDL, HDL, HbA1c, cysteine protease inhibitors cystatin endostatin C (CysC) and analyzed the relationship between ET, hs-CRP and its related metabolic factorsResult:①2 diabetes patients with hypertension group, the serum of patients with type 2 diabetes, ET, hs-CRP levels were higher than the control group (p<0.05),2 diabetes patients with hypertension in serum ET, hs-CRP levels than type 2 diabetes group (P<0.05).②DM merger HP group of 80 patients, hs-CRP-positive in 16 patients, accounting for 20.0%, the serum level of 6.46±1.47 mg/l, ET-positive in 25 cases, accounting for 31.8%, the serum level of 115.98±7.97 pg/ml; DM group of 96 cases, hs-CRP-positive in 15 cases,15.6%, the serum level of 3.48±1.12 mg/l, ET-positive in 23 cases, accounting for 23.9%; serum levels of 72.23±9. 17 pg/ml; normal control group of 60 patients, hs-CRP-positive in 3 cases, accounting for 5.0%, the serum level of 2.87±1.55 mg/l, ET-positive in 2 cases, accounting for 3.3%, the serum level of 58.18±7.81 pg/ml. DMEH group, DM group, hs-CRP, ET serum levels than those of normal control group, the differences between the groups was statistically significant (p<0.05); DM EH group hs-CRP, ET serum levels were significantly higher than the DM, differences between the groups was statistically significant (p<0.05).③DM merger HP group hs-CRP, ET serum levels and blood pressure, ankle brachial index, TG, CysC, HbA1c, FPG levels were significantly correlated (r= 0.644, P<0.05).④DM merger HP group determined the incidence of cerebrovascular complications was significantly higher than the DM group, respectively, in 25 cases (31.2%),16 patients (16.9%), statistically significant differences between (p <0.05).Conclusion:①DM merger HP, the serum ET, hs-CRP was significantly increased, indicating its possible participation in a type 2 diabetes, the incidence of hypertension development.②ET, hs-CRP-induced vascular inflammation can lead to multiple organ damage, kidney disease increases the performance of CysC levels and lower extremity atherosclerosis and decreased ankle-brachial index.③Blood pressure, TG, HbA1c, FPG poorly controlled, can promote serum ET, hs-CRP further increased, adding to its inflammatory vascular damage.④Hyperglycemia, hypertension coexist, the risk of cardiovascular effects have opportunity, atherosclerosis has increased significantly, which is the common variety of vascular inflammatory factors related to the role.
