Objective:Distal radial fractures is relatively common in clinical, which is higher in the emergency fracture patients, it mostly occurs in elderly patients, especially female patients in the majority. Along with the development of society today, the aging of the population is large, its rate is rising gradually. Meanwhile, fractures are mostly with soft tissue around swelling and pain, If processing inferior or improper, will affect fracture therapeutic effect, On the base of clinical studies, this topic use the method of maneuver reposition,external application of QingPeng ointment,Gesso splint external fixation,phaseⅲexercise and dietotherapy in treating patients with distal radius fracture. By the contrastive means of the remission of swelling,consciously pain and subcutaneous congestion, In order to provide a reliable clinical guiding recommendation and application of the treatment of distal radius fracture.Methods:According to the case diagnostic criteria,inclusion criteria and exclusion criteria, from June 2010 to February 2011,43 cases of patients with closed distal radius fracture were all chosed from orthopaedic clinic in Wuhan Integrated traditional chinese medicine and western medicine hospital. all fracture patients were divided into Colles fracture,Smith fracture and A2,A3,C1 by fracture displacement type and A0 type. Treatment group 33 cases were treated with the method of maneuver reposition external application of QingPeng ointment,Gesso splint external fixation,phase iii exercise and dietotherapy, control group 10 cases were treated with the method of maneuver reposition,Gesso splint external fixation,phase iii exercise and dietotherapy, both for six weeks' treatment. According to the remission of swelling,consciously pain,subcutaneous congestion and the changes of skin feeling, compared to its curative effect, To estimate the comprehensive effect and safety after the treatment.Using SPSS softwear to establish databases. Comparing one group itself before and after treatment with paired t test, the comparison among groups was performed with comparative t test. Measurement data was expressed by x±s.Results:Comprehensive therapeutic effect:19 cases cured,9 cases better,5 cases non-union in treatment group, with a total effective rate 84.85%; in control group,3 cases cured,3 cases better,4 cases non-union with a total effective rate 60%. The efficacy of the treatment group was better than control group(P<0.01).The comparison of reducing swelling and pain:The proportion of treatment group for the first day after the treatment was 45.45%, and 63.64% for the second day, the third day was 90.91%;The comparison of control group was 20% for the first day,40% for the second day, and 60% for the third day. The score of VAS was 3.56±0.68 for the third day in treatment group, and the fifth day was 2.64±0.64 in treatment group;and 6.86±0.78 for the third day in control group,5.50±0.48 for the fifth day. The improvement of VAS score in treatment group were significantly better compared with control groups (P<0.01).The comparison of reducing subcutaneous congestion:On the third day after treatment, the treatment group observed the proportion of subcutaneous congestion was 26.47%, including purpura larger proportion, and the control group observed the proportion of subcutaneous congestion was 50%, including petechia larger proportion; On the fifth day after treatment, the treatment group observed the proportion of subcutaneous congestion was 14.71%, including the proportion fell with ecchymosis and purpura, and the control group observed the proportion of subcutaneous congestion was 40%, including petechia a smaller proportion but a larger proportion of ecchymosis; On the seventh day after treatment, the treatment group observed the proportion of subcutaneous congestion was 2.94%, including the proportion fell with ecchymosis,purpura and petechia, and the control group observed the proportion of subcutaneous congestion was 30%, including the proportion fell with ecchymosis, but without changing proportion of purpura and petechia. Through three stages of subcutaneous congestion observation records, the treatment group were significantly better compared with control groups(P< 0.05), especially in the effect of eliminating petechia and purpura.Conclusion:The treatment of distal radius fracture by maneuver reposition,external application of QingPeng ointment,Gesso splint external fixation,phase iii exercise and dietotherapy perform well in detumescence,relieving pain,promoting blood circulation and removing blood stasis, it is a simple treatment for distal radius fracture, making good use of the advantages of combination of Traditional Chinese Medicine and western medicine, which proves to be an effective treatment and widespread application.
