Nursing Research On The Action Of Shenjue Separated Moxibustion In Treatingurinary Retention After Cholelithiasis Operation

Objective:To observe and compare the difference of emiction time and inducing emiction in urinary retention patients of cholelithiasis treated by operation using three different ways including shenjue separated moxibustion, traditional inducing emiction, and acupuncture with warmed needle, and explore the therapy ways that are true in clinical curative effect and are fit for nursing staff qualification operation and analyze the influencing factors of three intervention methods, which may provide the proofs for clinic and preliminarily reveal the action mechanisms of shenjue separated moxibustion in treating urinary retention of cholelithiasis treated by operation.Method:180 patients fitting for the standard of urinary retention after cholelithiasis operation were randomly divided into A, B, and C groups. Each group had 60 cases. The method of shenjue Separated Moxibustion was used in A group, Traditional inducing emiction in B group, and acupuncture with warmed needle in C group. Finally, total effective rate of emiction in 4h, the cases of voluntary micturition in 30min,60min,90min,120min, and 4h, first emiction time, mean emiction time, and mean emiction output were compared in three group patients.Results:(1) The comparison of total effective rate after intervention in 4h: the total response rate of A group was 88.3%, and C group was 91.7%. Their effects are better than that of C group(77.3%). There existed statistical significance among A group,C group and B group (P<0.05), while that of A and C group, No statistical significance was displayed(P>0.05).(2) The comparison of the cases of voluntary micturition after intervention in 30min,60min,90min,120min, and 4h:There are 67 cases displaying positive response in 30 min. Among these patients, there are 32 cases in A group,2 cases in B group and 33 cases in C group.There are 33 cases displaying positive response in 60 min. Among these patients, there are 15 cases in A group,3 cases in B group and 14 cases in C group.There are 22 cases displaying positive response in 90 min. Among these patients, there are 5 cases in A group,10 cases in B group and 7 cases in C group.There are 14 cases displaying positive response in 120 min. Among these patients, there are 1 cases in A and C group, respectively, and 12 cases in B group. There are 14 cases displaying positive response in 4 h. Among these patients, there are 0 cases in A and C group,17 cases in B group.There existed statistical significance in each time section (P<0.05).However, No statistical significance was displayed in A and C group(P>0.05).(3) Mean emiction time, and mean emiction output were compared in three group patients:No statistical significance was displayed in A and C groups(P>0.05). There existed statistical significance in mean Mean emiction time between B and A group, and B and C group.(4) Multiplicity:urinary output in A group was not influenced by multi-factor including gender, age, hospital days, operation time, postoperative days, the application of cholinergic drug, transfusion output in midoperation, the ways of operation, history of past illness, the style of hospital admission (P>0.05). while in B group, urinary output was affected by detaining urethral catheterization and the application of analgesia pump (P<0.05).In addition, urinary output was affected by age in C group (P<0.05)(5) The comparison of the time for the effect produce:In A group, effect produce was showed in 47 cases in 60 min, and the response rate was 94.3% in 53 total effect response cases. While in B group, effect produce was showed only in 5 cases, and the response rate was 11.4% in 44 total effect response cases. In C group, effect produce was showed in 47 cases, and the response rate was 85.5% in 55 total effect response cases.Conclusion:(1) shenjue separated moxibustion was used to treat urinary retention after cholelithiasis operation and its advantages are that curative effect is significant, effect produce is rapid, operation is easy, and patients are easy to be accepted. (2) Curative effect is similar between shenjue separated moxibustion and acupuncture with warmed needle treating urinary retention after cholelithiasis operation. Their total effect is better than that of inducing way.However, the nurses using acupuncture with warmed needle has not occupation qualification.(3) The application range of Shenjue separated moxibustion treating rinary retention after cholelithiasis operation is broad, which was not influenced by multi-factor including gender, age, hospital days, operation time, postoperative days, the application of cholinergic drug, transfusion output in mid-operation, the ways of operation, history of past illness, the style of hospital admission.(4) Attributing to Shenjue separated moxibustion belonging noninterven -tional therapy, which is fit for the range of occupation of nursing staff and do not need the operation of occupation doctor. So Shenjue separated moxibustion was favourable to be extensively applicable by nursing staff.
