BACKGROUND:"The method of eliminating slough and promoting tissue regeneration" is the main treatment methods in surgical treatmeat of traditional chinese medicine, it has been widely used to promote wound healing by doctors in all ages, especially in infectious refractory wound healing, its clinical effect is significant. Chinese medicine thought that "The new flesh wounldn't grow if the slough does not be eliminated" focusing on this theory, we think that the treatment to promot wound healing of tissue regeneration should eliminat slough first. At present the application of this method have some.problem:the prescription drugs are used confused; pharmaceuticals are too variety; the dose of chinese herbal medicine is priscibed without uniform standards, and so on.This topic is based on the clinical experience summary of my teacher professor Li guodong and the older generation of Anorectal experts in Guang An Men Hospital, and the pre-paste preliminary study of ShengJiYuHong Ointment(SJYHO), we research the use of SJYHO with 15% ShengJiHongFen Ointment(SJHFO). We assess the efficacy of "eliminating slough" and "promoting tissue regeneration" in the method and preliminary study in its mechanism. The study including two parts: clinical and animal experiments. During,the study, we use the Chinese medicine theory and methods of modern medicine.OBJECTIVE:To observe the clinical efficacy and safety of "The method of eliminating slough and promoting tissue regeneration" used in infectious refractory wound healing by clinical reserch. To observe the mechanism of the method's action through the animal experiments. Preliminary study of mechanism of the method's action. To explore a more safe and effective treatment on promote wound healing of postoperative infection. To provide data of clinical efficacy and basic research of the method.METHODS:Clinical study:90 cases of perianal abscess and anal fistula were enrolled in the study, and randomly divided into 3 groups:groups A (30 cases of SJHFO), groups B (30 cases of SJYHO), groups C (30 cases of Vaseline). We observe the wound healing time, persentage of healing areas afer 7,14,28 days from use the ointment, color changes in the wound, PH changes of wound and other indicators. To evaluate the effect of "The method of eliminating slough and promoting, tissue regeneration" used in the clinic.Group:group A:Using 15%SJHFO dressing until the wound turn to fresh and bright red color then change to use SJYHO dressing, once daily, group B:using SJYHO dressing, once daily, group C:using Vaseline dressing, once daily.Experimental studies:Animal experiments were divided into 3 groups: groups A (15% SJHFO with SJYHO); groups B (SJYHO); groupsC (Vaseline group). After manufacturing rabbit skin-deficit infection models, we dressing the different group's wound in different way. observe wound's PH, protein content and the persentage of healing areas in 3,7,14,21 days. We measure the same dosage and different kinds of durgs'including 15% ShengJiHongFen Ointment, ShengJiYuHong Ointment, Vaseline, Erythromycin Ointment andTai ning Ointment antibacterial ring which affect on, different kind of germ:Staphylococcus aureus, Coccus epidermidis, Pseudomonas aeruginosa, Streptococcus agalactiae, Escherichia coli and Proteus to observe the antibacterial effect of different drugs.RESULTS:Clinical study:After 4 weeks from dressing wound,3 groups'healing ratio were:93.3% by 15% Sheng JiHongFen Ointment,80% by ShengJiYuHong Ointment and 66.7% by Vaseline. The effective ratio were 6.7%,20% and 30%. After 1 week and 2 weeks from dressing wound,3 groups were significant difference in healing ratio and healing time. After 3 days from dressing wound the group of 15% SJHFO were significant difference than other groups in the wound's color,the amoun of wound's secretions and the changing of wound's PH. The study also shows a significant difference in the time of eliminating slough and tissue regeneration. The time of the group of 15% SJHFO were shorter than the others. The three groups were not occurring Adverse drug reaction.Experimental Research:Animal experiments:After 3 days from dressing wound the wound's PH of the group of 15% SJHFO were lower than the group of SJYHO, the group of SJYHO wound's PH were lower than the group of Vaseline. Determination of secretions'protein content in each group showed there were no significant difference between each two groups. Bacteriostatic experiment:The results showed that the 15%SJHFO were effectual, on six kinds of bacteria. Especially obvious effect on Bacillus bacteria. It was significant difference than the SJYHO. The antimicrobial effect by 15%SJHFO group on Pseudomonas aeruginosa were weaker than the Erythromycin Ointment group andTai ning Ointment group.15%SJHFO group have a little effect on coccus. The effect was obviously stonger than the SJYHO group but weaker than the effect on Staphylococcus aureus by Erythromycin Ointment group and effect on Coccus epidermidis and Streptococcus agalactiae by Tai ning Ointment group.Findings: The method of eliminating slough and promoting tissue regeneration can speeds up the injured area cicatrisation, through the methods of the seceretiom of wounds'adidification, strengthens the andt-infection function, the rise of the content of protein in wound tissues to achieve the goal of promoting the cicatrization of the injured area faster.The therapy is very fit to dressing change for some infection operation wounds such as anal fistula and anal edema.15%SJHFO and SJYHO are very safely for patients in clinic. We draw the conclusion according to animal experiment results that the therapy possess following functions, such as detoxication, removing dough and promoting regeneration of tissue.The reseaach was expected to provide theory fondation of the method, eliminating slough and promoting tissue regeneration, on accelerating the wound healong.At the same time, it is obvious science evdience that the producets was seceritily used in clinic.
