Background:Anal fistula is a familiar colorectal disease,the only method to cure is operation.The healing time of the wound is long because of the particularity of the disease location.Inappropriate nursing,unexpedited drainage or poor capacity of tissue repairment could leads to bad healing or recrudescency,this could increase the pain and costs of patients.The methods to promotes wounds healing are variety,but there are some flaws and inadequate of these methods.Objective:The study is a randomized controlled clinical research,comparing with the control group using growth factor,evaluating the efficacy and safety of the method of eliminating slough and promoting granulation to wound healing of anal fistula,exploring more safe and effective treatment methods to the promotion of anal fistula wound healing and providing basic data of clinical efficacy.Methods:The study included 70 eligible patients between April 2007 and April 2008,and randomly divided into test group(eliminating slough and promoting granulation group,QF group,N=35)and control group(βFGF group,BF group, N=35).The QF group using Xiaoyan gauze and Shengji gauze to Dressing,and the BF group usingβFGF and sterilized vaseline gauze.To compare the wound area,wound healing rate,wound secretion,wound tissue form and histology examination,the time of slough fall off,healing time,hospitalization, costs,and safety examination of two group's patients in third day,seventh day,fourteenth day after operation.All the data analysis using SPSS13.0, enumeration data was tested by x~2;comparison of the measurement data between the samples was tested by t in both independent samples;ranked data between the samples.Results:There are one patient in test(QF)group and two patients in controlled (BF)group exit the study because of adverse reactions.Sex,ages,wound area, wound secretion score,wound tissue score were similar between the two groups before treatment.1.Wound area and healing rate:BF group excelled QF group in seventh day, the difference is significant(P<0.05),the third day and fourteenth day have no significant difference(P>0.05);2.Wound tissue form,histology examination and wound secretion:QF group is excelled of all above in third day(P<0.05),the seventh day have significant difference in wound tissue form and histology examination of two groups(P<0.05),the wound tissue is more white and hard in BF group.The granulation growth of BF group is faster than QF group in fourteenth day(P<0.05),but most of them were pale,the secretion have no significant difference between two groups;3.Effect index of two weeks after treatment,clinical effects,the time of rots fall off and wound healing time:the effect index of two weeks after treatment,the time of rots fall off and wound healing time have no significant difference between two groups(P>0.05).The total curative effect is 100 %in two groups;4.Hospitalization and costs:QF group were less than BF group(P<0.05);5.Safety and adverse events:There were no obvious abnormality in blood, liver and renal function before operation and a week after operation.The adverse events appear in one patient of QF group and two patients of BF group.Conclusion:1.The test(QF)group and controlled(BF)group are effective in promoting wound healing after the operation of anal fistula;2.The method of eliminating slough and promoting granulation could effectively shrink wound area,reduce healing time,protect wound granulation,and promote granulation;3.The method of eliminating slough and promoting granulation is eliminating rot and pus in early,then the effect of promoting granulation is stronger;4.The method of eliminating slough and promoting granulation is a safe,economical method.
