Objective: To study the changes of plasma levels of Urotensin II (UII),adrenomedullin(ADM) in patients with diabetic foot,to study its posible role in the development of diabetic foot and to study the influnce of PrG on plasma levels of UII,ADM in patients with diabetic foot.Methods: According to 1999 years WHO diabetic standard,60 cases type 2 diabetic patients were selected, who were hospitalized in Endocrine Department of Jiamusi University first Affiliated Hospital from August,2006 to September,2007.Including 30 cases DF (19males,11 females),age is from 30 to 76(57.50±11.53) years old. 30 cases type 2 simple diabetic patients (16males,14females)were randomly selected at the same time,age is from 30 to 75(55.73±10.18).The cases were excluded serious diseases of heart,head,lung et al. And 30 cases nomal control subjects (15males,15females),age is from 33 to 68(54.23±8.64).The plasma levels of UII and ADM were measured in patients with diabetic foot group before and after treatment with PrG,simple diabetic patients group and nomal control group.Plasma UII,ADM concentrations were measured by radioimmunoassay.Results:1.The plasma levels of UII were 1.72±0.14pmol/L,the plasma levels of ADM were 15.02±3.08ng/L in nomal control group. The plasma levels of UII were 3.87±0.92pmol/L, the plasma levels of ADM were 26.45±1.90ng/L in simple diabetic patients group. The plasma levels of UII were 5.26±0.97pmol/L, the plasma levels of ADM were 33.14±2.17 ng/L in patients with diabetic DF before treatment,The plasma levels of UII,ADM were significantly higher in patients with diabetic foot than that of simple diabetic patients group without DF (average P<0.01).2.Paients with diabetic foot grade II (19cases): The plasma levels of UII were 4.69±0.58 pmol/L, the plasma levels of ADM were 44.57±4.21ng/L, grade III (7cases): The plasma levels of UII were 5.84±0.24 pmol/L, the plasma levels of ADM were 49.82±1.57ng/L, grade IV (4 cases): The plasma levels of UII were 6.98±0.50 pmol/L, the plasma levels of ADM were 57.16±4.80ng/L. With the condition more serious in patients with DF,the plasma levels of UII and ADM increased more significantly: grade II < grade III < grade IV (average P<0.01).3.A positive correlation was found between plasma levels of UII and of ADM (r=0.576,P<0.01).4.The plasma levels of UII and ADM (UII: 4.60±0.47pmol/L,ADM: 47.50±5.81ng/L)were significantly decreased After treatment with PrG in patients with DF (P<0.01).Conclusion:1. The plasma levels of UII more significantly increased in the patients with DF than that in simple diabetic patients group without DF.With the condition more serious in diabetic patients with DF,the plasma levels of UII increased more significantly.This indicates the increase of UII involved in the pathophysiologic process of diabetic foot.2. The plasma levels of ADM more significantly increased in the patients with DF than that in simple diabetic patients group without DF.With the condition more serious in patients with DF,the plasma levels of ADM increased more obviously.This illuminates the increase of ADM involved in the pathophysiologic process of diabetic foot.3. A positive correlation was found between plasma levels of UII and of ADM.This indicates the increase of Plasma ADM is related to the secretary increase of UII in patients with DF,which is a compensatory protectant mechanism.4. The plasma levels of UII and ADM were decreased after treatment with PrG in patients with DF,which can help recovery from affection of diabetic foot.5.Recover the balance between the plasma levels of UII and ADM may offer a new approach for patients with DF.
