Objective To investigate the MRI feature of cerebral infarction with diabetes mellitus and to provide clinical index for the prophylaxis and treatment of ischemic cerebrovascular disease.Methods 142 cases of cerebral infarction were enrolled .All the patients with cerebral infarction were devided according to the combination with or without diabetes and hypertension into three groups :34 patients witn diabetes, 54 patients with hypertension and 54 patients with hypertension combined diabetes . The SBP,DBP,BMI were recorded ; the level of FPG,FIB,TC,TG,HDL-C,LDL-C,lp(a) were measured in the three groups.The size , number, location of the infarction and the major cerebral arterial territories of the ischemic lesions were detected by MRI in the three groups.Results 1) The SBP DBP in the hypertensive diabetic patients cohort were higher than the cure diabetes series and cure hypertension series.the level of FPG in the diabetes and hypertensive diabetic patients were higher than hypertensive patients.There were no significant difference in the level of FIB,TC,TG,HDL-C,LDL-C,lp(a) among the three groups .2)There were no significant difference in the location of the ischemic lesions among the three groups.but most of the soci were note in the nucleus ganglion and corona radiata. Most of foci were multiple in the three groups and the middle cerebral artery territory is the most frequently involved, but a higher proportion in types of multiple lacunes and a higher proportion multiple territories were in the hypertensive cohort and hypertensive diabetic patients compared with the diabetes group (p<0.05).While there were no difference between the hypertensive group and the hypertensive diabetic group .Conclusions 1 The main type of infarction was multiple lacunes in the patients with diabetes mellitus ,.The middle cerebral artery territory is the most frequently involved. The most frequent locations were the nucleus ganglion and corona radiata. Hypertention was the most important risk factor for lacunes ,while diabetes mellitus plays an important role in the etiology of disseminated cerebral small-vessel diease.2 Diabetes and Hypertension increase the risk of stroke independently, but a synergistic effect between hypertension and diabetes mellitus was dot found in MRI.3 The patients who has no Hypertention should be watched out for diabetes mellitus while their MRI show multiple lacunes.
