Baotou City Residents Demand For Health Services And Resource Allocation

In order to provide information for the regional health planning (RHP) in BaoTou city ,this study ,by surveying health care services in a mass of 2305 persons from 792 random sample households in BaoTou city ,was trying to know the problem of the health and demand for medical care of the inhabitants and status of supplying and demand of health resources ,studying the factors influencing people's morbidity and analysing the extant of health services use and the factors influencing the utilization.Problem of health : The study investigated the "two week prevalence rate of illness" and the "prevalence rate of chronic diseases" of the samples, which is 8.46% and 8.63% respectively .the main reasons of the incidence of two week are cold and chronic disease .The main diseases affecting the inhabitant's health are hypertension ,vascular disease ,chronic gastroenteritis ,rheumatic arthritis and the disease of brain blood vessel . Non-conditional Logistic analysis indicates that the main factors associated with the diseases are age ,marital status and health insurance.Utilization of outpatient services :The incidence of outpatient services was 5.16% in the investigated mass ,54.34% in patients and there were 45.66%of the patients who did not seek care because of self-medication(39.0%),mild disease(22.0%) and economicdifficulties(18.0%) .It was known that the main factors affecting the utilization of single factor and multiple factor analysis were age , marital status ,educational levels ,average income and days the disease lasted.Utilization of inpatient services : The recall period of use of inpatient services was one year .The hospital admission rate at that period was 4.12% .For patient who were in hospital were mainly suffering from circulatory system disease ,respiratory system disease ,nerve system disease ,digestive system disease and injury and poison. There were 65.26% of the patients who utilized the services provided by higher hospital. Demand for inpatient service are unreasonable .Factors mainly affecting the utilization of inpatient services studied with the method of single factor and multiple factor analysis wee age ,average income and health insurance .The patient who did not hospitalized were because of economic difficulties(58.8%) ,mild disease (26.5%) and no-time (5.9%) .The incidence of "noninpatient among the investigated mass was 26.36% and that reflected the deficient utilization of inpatient services .Demand for medical care and health resources : People has great potential demand in medical care .Medical care in different levels are unreasonable. Total health resources of doctors and hospital beds are superfluously ,exceptthe nurses which are balanced on the whole .Extent for demand of health resources are different in different level of the medical institutions .The resources lack in primary medical department while superfluoused in high medical department.Advice based on this study : Controlling the total of health resources in allocating ,adjusting the structure and function of health service ,and accelerating the utilization of the resources .strengthening the city's Community Health Services ,Improving the quality of health Services .Improving the quality of health technology staff.
