Objective:This study selected rural residents in Yongxin County,southern Jiangxi Province as the research object.Through the longitudinal comparison between 2013 and 2016,to understand the changes in the demand and utilization of health services in rural areas of Yongxin county.Analyze and grasp the characteristics of rural residents' health service demand and utilization.Assessing the growing demand for health services for residents,and put forward relevant countermeasures and suggestions to provide scientific basis for the scientific and rational formulation of health policy and the effective allocation of health resources in Yongxin county.Methods:Using a stratified cluster sampling method,residents of 9 village committees in 3 townships in Yongxin County were randomly selected.A questionnaire survey was conducted on the status and related factors of rural residents' health service by means of door-to-door investigation.The health service needs of rural residents and the utilization of health services were obtained.The collected data were statistically analyzed by using the Chi-square test and binary logistic regression analysis.Results:(1)The two-week prevalence rate of rural residents in Yongxin County in 2013 was 29.5%.In2016,the two-week prevalence rate of rural residents in Yongxin County was 32.2%.The top four diseases before two weeks of illness are: dizziness/headache/tinnitus,acute and chronic gastroenteritis,waist pain,cold cough.There was no change in the prevalence and prevalence in two years and two weeks.Multi-factor analysis shows: two weeks of sick women are higher than men;The older,the higher the risk of illness;The two-week prevalence is highest in other occupations without fixed work.(2)In 2016,the prevalence rate of chronic diseases among rural residents in Yongxin County was 37.5%,which was higher than that of 34.3% in 2013(P<0.05).The standardized rate of the age group above the age of 15 is basically the same as the national level;The composition and ranking of chronic diseases did not change.The top four are: hypertension,bile/urine stones,chronic gastroenteritis,cervical and lumbar disease;Multi-factor analysis shows: The prevalence of chronic diseases was higher among women,older people,widowed divorces or separated persons.(3)In 2016,the standardization rate of rural residents in Yongxin County for two weeks was13.7%,which was higher than that in 2013(12.0%)(P<0.05);In 2013 and 2016,residents who visited the township hospitals or village clinics for two weeks were 46.8% and 55.6% respectively.Multi-factor analysis shows:The consultation rate in 2016 was higher than in 2013;Women are higher than men;Older people have a high rate of visits;The highest number of other professionals is free of fixed jobs;The other attendance rates are low at their own expense.(4)The standardized hospitalization rates for rural residents in Yongxin County in 2013 and2016 were 8.4% and 8.2%,respectively,and there was no difference in hospitalization rates for two years(P>0.05),both lower than the national average(9.0%);In 2013 and 2016,51.7% or58.3% of hospitalized patients should be hospitalized for financial difficulties.Multi-factor analysis shows: the hospitalization rate of women was higher than that of men;Older people are taller;the rate of hospitalization for widowed or divorced or separated people was high;There was a higher rate of other occupations without fixed jobs and higher rates of hospitalization for new rural cooperatives Conclusion:The prevalence rate of rural residents in Yongxin County was higher than that of the national rural average in two weeks.The chronic disease rate and the two-week consultation rate are basically the same as the national rural average.The utilization rate of in-patient health services was lower than the average level in rural areas in China;The outflow of outpatient visits is still unreasonable,and the role of grassroots health agencies had not been fully utilized;Economic difficulties are the main reason why residents should be hospitalized without hospitalization.Focus should be placed on the health services needs of women,the elderly,widowed divorces or separated persons.In view of the characteristics of residents in Yongxin County,it is suggested that health care should be increased for special groups such as the elderly and women.Strengthen the prevention and control of chronic diseases,combine regional and population characteristics,and implement key population intervention strategies for people of different age groups and different marital status;To take advantage of the geographic advantages of grass-roots health institutions,improve the construction of rural tertiary medical service network,and focus on improving hospitalization service capabilities.
