With External Fixation Treatment Of Intertrochanteric Fractures Of The Clinical Studies

Objective To summarize the therapeutic experience and analyse the clinical effects of anatomical plate and combined external fixator for the elderly intertrochanteric hip fractures.Methods From Oct.2006 to Jun.2009,42 cases intertrochanteric hip fractures were treated.19 cases were treated with combined external fixator(combined external fixator group), including 7 males and 12 females aged 71-103 years. According to Evans classification, there were 3 typeⅠ,5 typeⅡ,6 typeⅢ,5 typeⅣ;and 23 cases were treated with anatomical plate (anatomical plate group) including 9 males and 14 females aged 73-97 years. There were 5 typeⅠ,4 typeⅡ,10 typeⅢ,2 typeⅣ,2 type V.All patients have Jie gu qi li pian after operaiong.Comparisons and statistical analysis were made in operation time, amount of bleeding during operation, hospitalization time, the time of loaded walking, healing time and treatment effect.Results combined external fixator group operation time(29.368±3.2525)min, amount of bleeding during operation (30.526±5.2427)ml, hospitalization time (4.3684±1.1648) d, the time of loaded walking (14.526±1.8669) w, healing time (14.632±2.7977) w, and the anatomical plate group were (58.987±8.5996)min, (256.09±30.112)ml, (13.783±1.9761)d, (11.391±1.6442)w, (11.391±2.3272)w, respectively. All the cases were followed up for 6 months to 15 months with an average of 8.6 months. According to Harris score criterion, the combined external fixator group mean scores were (86.368±3.6395) points. And anatomical plate group (86.435±4.1977) points. The clinical effects of anatomical plate and combined external fixator for the elderly intertrochanteric hip fractures has no significant difference (P>0.05).There were significant differences in the operation time, amount of bleeding, hospitalization time, the time of loaded walking and healing time (p<0.05)Conclusion The author considers that the effect of combined external fixator and anatomical plate for the elderly intertrochanteric hip fractures are both satisfactory and each has its own merits. Combined external fixator has advantage such as less complication and operative injuries than that of anatomical plate which is more suitable for the weaker with elderly intertrochanteric hip fractures. "Jie gu qi li pian" can reduce low pain and speed up healing of bone fracture.The orthors conclde that:the therapy of Combined integrating external fixator and Chinese medicine are suit for the elderly intertrochanteric hip fractures.It is worthy of going into clinical study.
